Breast Augmentation with Silicone Implants

47 years old, 3 months postop.  400cc silicone breast implants, submuscular, inframammary approach.  When large implants are used with small breasts, it is best to use silicone implants.  They feel and look much more natural and are always the best choice in thin patients with less breast volume to start with.  This patient did not have the underarm approach because of the larger silicone implants.  Silicone implants require a larger incision than saline, and the larger incision is hard to conceal in the underarm.  Axillary augmentation is used for implants smaller than 400cc.
