This patient had a previous breast augmentation with saline implants. She was diagnosed with invasive ductal carcinoma in her left breast. Instead of just a left sided nipple/areola sparing mastectomy, she opted for a bilateral mastectomy, because her gene testing was positive. She had immediate direct to implant reconstruction. This was a more challenging customized case due to her having implants already placed above the muscle. After her general surgeon performed the mastectomy, Dr. Janssen removed the intact saline implants, and used Allergan 410FF 655mL implants with Alloderm. The patient was aware that the bilateral operation would result in less rounded appearing breasts than she previously had. The patients thought that the 410 implants were too stiff, and had them later replaced with medium height, moderate profile Mentor implants 640mL. She also had lipostructure to the upper poles of both breasts.
