This Korean American patient requested a more western look. She had trouble applying makeup. The epicanthal skin folds rested on her eyelashes, deflecting them down. These folds were released using small interpositional skin flaps (Z-plasties). The caruncles are now visible in the inner corner of her eyes, which appear more open. Her sutures were removed 4 days after surgery and she returned to work with makeup 6 days after surgery.
