A mother of 3 in her early 30’s who is bothered by bulging and laxity of her abdomen, as well as drooping and deflation of her breasts. She is 5’ 8 ½ “ and 129 lbs. She is shown before and again, 7 months after bilateral augmentation mastopexy combined with abdominoplasty with “floating” of the umbilicus. 350 cc high profile plus silicone gel implants were used in a “dual plane” position. Her case shows us several important nuances. When the nipple is at or below the breast fold (you can put a pencil under your breasts and look in the mirror) then a breast lift is almost always needed. The implants add the volume, and because her breasts are looser, the high profile implant helps to fill up the extra space but without pushing too much against the breast tissues and looking unnatural. A vertical mastopexy was performed, with removal of skin from around the areola and lower breast, leaving a “lollipop” scar. Her umbilicus is relatively high and her upper abdominal skin is in good condition. So we can detach her umbilicus from the abdominal wall and “float” it down as the skin and underlying muscles are tightened.- in this way she does not have a scar around the umbilicus that is seen with a traditional abdominoplasty. Scars will typically continue to fade and improve over the next 18 months. Diver's view shows nice tightening of the loose skin.
