This patient had a very clear, specific goal in mind. He wanted to have smaller nostrils, since he thought his were looking too flared all the time.
Some nostril flare is important to have and actually looks good on faces, so such an operation needs to be delicately handled. In this particular patient's case, I agreed that his own flare could be "toned down" a bit, just like he wanted, since it was noticeably prominent.
