57-year-old patient presented with T1 N0 right breast cancer. She underwent bilateral mastectomy and immediate reconstruction utilizing a portion of the latissimus muscle and a tissue expander. The dissection was all done from the mastectomy incision without any back scars. 9 months later the expander was exchanged for a high profile silicone implant of 400 cc. Note that there are no back scar on the back view.
