AMaryland woman had a droopy tip of the nose when smiling. Active smiles may pull the nose down to a large degree. She underwent finesse rhinoplasty to improve facial balance via an endonasal approach, without any external incisions. The tip of the nose is in a better position and does not droop anymore. She also had deviated septum surgery (septoplasty) to improve nasal breathing. The patient declined reduction of her nasal hump.
