This is a 30 year old female who underwent rhinoplasty overseas some 10 years earlier. She presented to me with a pinched tip, dorsal hump and overly rotated nose. Functionally, she suffered from both internal and external valve collapse. After consultation she decided to go proceed with an aesthetic reconstructive rhinoplasty whereby the dorsal hump was resected, spreader grafts as well as lateral crural strut grafts were inserted and the tip was de-projected and counter-rotated through a medial crural tuck up technique. A temporalis fascial blanket was inserted to camouflage any pre-exisiting bony irregularities.
