CONSULT GIFT CARD PROGRAM TERMS AND CONDITIONS


This Consult Gift Card Program (this “Program”) rewards RealSelf, Inc. (“RealSelf”, “us”, “our” or “we”) users (“User” or “you”) in select circumstances. Your participation in the Program is subject to these terms and conditions (these “Terms”). It is your responsibility to read and review these Terms prior to participation in the Program.


  1. Program Summary. The Program rewards Users with a VISA Gift Card (the “Gift Card”) of up to one hundred dollars (US$100) for each “Qualified Consult”.  A Qualified Consult means: 

(a) User requests a consultation with a health care provider (“Provider”) through the RealSelf platform [https://www.realself.com/] (a “Consultation”)

(b) User attends the Consultation and pays the Consultation fee, which may exceed $100; and 

(c) User provides RealSelf with a copy of the receipt confirming proof of payment for the Consultation (the “Receipt”), sent via email.  


  1. Eligibility. The Program is available only to legal United States residents, 18 years of age or older at the time of the Consultation. Void where prohibited by law. Each request for Consultation must be submitted between May 19, 2025 and June 4, 2025 on RealSelf.com, via either the “Request a Consultation” contact flow or by clicking to call from the provider’s RealSelf profile. Consultation requests submitted directly to Provider or through their website are not eligible. The Consultation itself must be completed no later than August 31, 2025.  One Gift Card will be awarded per Consultation, with a maximum of two (2) Gift Cards per individual. Each Gift Card amount will be the lesser of $100 or the amount paid for the Consultation. Gift Cards will be awarded while supplies last. The Program is not available where: (a) your procedure of interest is reimbursable, in whole or in part, by any federal healthcare program, including Medicare, Medicaid or TRICARE; (b) your procedure of interest is reimbursable, in whole or in part, by any public or private insurance; or (c) you are a healthcare professional.


  1. Receipts. Each Receipt must be sent to RealSelf from the same email address used to submit the Consultation request. The names of the User and Provider or medical practice providing the Consultation must be clearly visible on the Receipt. User must submit the Receipt to consultoffer@realself.com within 60 days of the consultation date.


  1. Gift Cards. For each eligible Consultation, a digital VISA Gift Card reward link will be sent to the email address used to submit your consultation request. Gift Cards may not be redeemed for cash. The maximum amount of Gift Cards you may earn via the Program is $200. Gift Cards will be processed in the order information all necessary information is received. Please allow up to two (2) weeks for Gift Card processing.


  1. Termination. We may terminate your participation in this Program and/or cancel the entire Program at any time, at our sole discretion. In such circumstances, these Terms will continue to apply to Qualified Consults made prior to such termination or cancellation, unless your participation is terminated because of a violation of these Terms, or because of a change in law, or change in interpretation of law, renders the Program or your participation in the Program unlawful. Failure to comply with any of these Terms, or any fraud or abuse relating to participation by you or anyone acting on your behalf may result in the immediate termination of your participation in the Program, the closure of your RealSelf account and/or forfeiture of any outstanding rewards. We reserve the right to review and investigate all activities and to suspend or modify your access to the Program indefinitely, in our sole discretion.


  1. Acceptance of Terms; Entire Agreement. By participating in the Program, you agree that you have read, understand, accept and will be bound by these Terms. These Terms constitute the entire agreement with respect to your participation in the Program.


  1. Disputes. This Program will be governed by the laws of the State of Washington, without regard to otherwise applicable conflicts of law provisions. Any dispute will be submitted to the federal or state courts located in King County, Washington. YOU AND WE WAIVE ANY RIGHT TO A TRIAL BY JURY FOR A DISPUTE RELATED TO THESE TERMS OR THE PROGRAM.


  1. Disclaimer. RealSelf is neither a physician nor a healthcare provider and is not providing medical advice or treatment of any kind. Any health or medical information provided by RealSelf is for informational purposes and shall not be construed as medical advice, diagnosis or treatment. PARTICIPATION IN THE PROGRAM IS AT YOUR OWN RISK. THE PROGRAM IS MADE AVAILABLE TO YOU ON AN “AS IS” BASIS WITHOUT WARRANTY OF ANY KIND, EXPRESS OR IMPLIED.


  1. Severability; No Waiver. If any provision in these Terms is held to be invalid, void, or unenforceable, such provision (or the part of it that is making it invalid, void or unenforceable) will be removed but will not affect the validity and enforceability of the remaining provisions.  We shall not be deemed to have waived any of our rights or remedies hereunder unless such waiver is in writing and executed by our authorized representative. No delay or omission by us in exercising any rights or remedies shall operate as a waiver of such rights or remedies or any other rights or remedies.
