Why aren't stateside surgeons assisting with aftercare or issues for medical tourism?

Stateside doctors warn us about going abroad for surgery. It makes perfect sense as far as the aftercare...but why are there no specialists catering to the aftercare market? Instead of just working against it, why not accept that there are docs abroad getting the results women want for the price we feel is appropriate? Why not have these women sign a release and help them out with the random infection or even repair? Sounds like docs are missing out on a secondary market. What do you think?
