How did you notify your employer?

I'm sure this may be a "duh" kind of question for a lot of people...but it's something I've been worrying about. I have my surgery date set for Nov 30 (just over a month from now). I almost never take time off (yeah, I'm lame/boring like that, and rarely get sick enough to call in). I work for a very small company, I'm the only one in my dept. I know it shouldn't be a big deal to just say "hey, I need a few sick days on these dates", but I'm totally stressing over it, and way over thinking it. Maybe it's because my boss is male, and this is rather personal. (Plus I work in a very male dominated field) Maybe it's simply that once I request time off it makes it set set. No take backs level of real. So how did you notify/request leave? How much info/detail did you give?
