I am completely underprepared for this Breast Reduction journey!

Hey everyone. Man do I feel underprepared for this journey! So first off, I'm 35, no kids, 5'8" , weigh 145 and have size 32G breasts. I asked three different doctors at three different hospitals years ago if they would do the reduction on me, and 2 said they would. The third said he would do liposuction, because I don't scar very well, but I never went through with it because of lack of insurance/lack of money in general. I don't remember the name of the doctors though. I've been looking through insurance papers for the requirements online, and they all want you to be over 18 (check), have enough breast tissue removed according to the Schnur Sliding Scale (which wouldn't be a problem, so double check), but what I don't have is supporting paperwork from other doctors, like a chiropractor, my PCP, or a physical therapist. I could probably go see these doctors and get them to write in to my insurance and say they think I should get the operation done, but I need at least six months to a year of documented pain, which I don't have officially. (Even though I live with it every darn day!) So the question is this: Did anybody get approved by their insurance without any other doctors letters other than your plastic surgeon? Or maybe your plastic surgeon and your primary care doctor? Because there's no point in me going to see a plastic surgeon and paying for a consult if I'm just going to get denied by my insurance since I don't have the appropriate existing paperwork of documented pain.
