No arch in lower incisors after 18 months of treatment. Should I be concerned?

I've been in Invisalign treatment for over 18 months for crowded teeth and a rotated upper left eye tooth and lower left eyetooth with twisting lower incisors. The original treatment course was 29 trays at 2-week intervals. The refinement has been 18 trays at 10-day intervals. With two trays to go, I am concerned that the alignment of my lower incisors are in a diagonal straight line (moving inward from my left side to my right.) My dentist (who is a Premier Provider) says that she will fix this with enamaloplasty, but I can't see how cosmetically grinding teeth down will correct the lack of arch.. I am so disappointed that I may have to go through yet another refinement (two were included in my plan), but am not willing to "settle" for the current look of my lower teeth after all this time, effort, discomfort and inconvenience. Comments and suggestions appreciated.
