Dr. William A. Portuese is a double board-certified facial plastic surgeon with over three decades in practice. He is the founder and medical director of the Seattle Facial Plastic Surgery Center and the Portland Center for Facial Plastic Surgery in Portland, Oregon. Widely considered one of the premier facial plastic surgeons in the Pacific Northwest, Dr. Portuese specializes in all areas of aesthetic facial surgery, including facelifts, neck lifts and liposuction, blepharoplasty, rhinoplasty (including revision rhinoplasty), and forehead lifts, as well as cheek and chin implants. “Because of my double board certification, I can extend a high level of expertise, mastery, and professionalism to my patients,” says Dr. Portuese. Dr. Portuese earned his Bachelor of Science degree and his medical degree from Oklahoma State University. He completed an internship in categorical otolaryngology–general surgery and residency in otolaryngology–head and neck surgery and facial plastic and reconstructive surgery at Louisiana State University Hospital, followed by a year-long fellowship in facial plastic and reconstructive surgery at the renowned Lasky Clinic in Beverly Hills, California. “Early in my career, I sought out the best of the best surgeons doing facial cosmetic surgery, specifically facelifts, eyelid lifts, and rhinoplasty,” says Dr. Portuese. “I observed and assisted these master surgeons performing their craft, and that expertise is what I bring to my practices in Seattle and Portland.” Dr. Portuese is board-certified by both the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery. A: Your eyes and cheeks look just fine, it does not appear that any work needs to be done. For a rhinoplasty evaluation, a full set of facial photographs from all angles(Not just from the front) are going to be required to make a determination about how best to proceed. A closed rhinoplasty approach can accomplish shaving down the dorsal hump, narrowing the bridge line and refinement of the nasal tip, with all of the incisions placed on the inside of the nose. Digital computer imaging of your nose upon your facial features would also be helpful to understand what can and cannot be accomplished with the procedure.

A: The limited photographs demonstrate fat deposits located both above and below the platysma muscle in your neck. A neck lift procedure, accomplishes removal of the fat deposits above the muscle also with liposuction, and surgical extraction of the fat deposits below the muscle, which also includes a platysma plasty to significantly improve the jawline. Liposuction alone will not be very effective. Before entertaining any surgical procedures in the neck, your BMI should be 32 or less for best results.

A: A closed rhinoplasty approach can accomplish shaving down the dorsal hump, which is composed of both bone and cartilage. Osteotomies placed in the nasal bones are also required to close the open roof created from the hump removal itself. You may also need a minor tip adjustments to make sure the tip is balanced with the entire new nose in a three-dimensional fashion. Digital computer imaging would be helpful to understand what can be accomplished with the procedure. Rhinoplasty is the most difficult procedure to perform correctly in the entire field of cosmetic surgery, so choose your surgeon wisely, based on extensive experience producing natural results.

A: A full set of facial photographs from all angles that are in focus, are going to be required to make a determination about how best to proceed. It's also important to obtain copies of the operative report to find out what was accomplished in the first procedure. If there's a dorsal hump present, that can be filed down. Osteotomy's are performed when there's a wide bridge line or an inverted V deformity. It's also important know how much cartilage is left over on the inside of the nose for grafting purposes, since that cartilage is used for grafting purposes to help with the inverted V deformity.

A: From the photographs presented, you have an over projecting nose, and an under projecting chin. Give strong consideration to placement of a chin implant to augment your chin forward for better facial balance and proportions, especially with respect to your nose. You can project the chin further outward with a chin implant procedure, than you can de-project the nose backward with a rhinoplasty procedure. Revision rhinoplasty is more difficult and less predictive than a primary rhinoplasty. It's also very difficult to decrease the overall projection of the nose and keep the entire nose itself looking great in a three-dimensional fashion.
