A: Hi and thanks for your question. Based on your photos. You need a breast-lifting surgery. After breast lifting you get lifted but slightly smaller breasts than now. My first recommendation is breast lifting and implant for you. With this combination you can get lifted, round and attractive breast. After this surgery you have incision around areola and from the areola to bra sulcus. The operation takes about 2.5 - 3 hours. You should use a sport bra for 6 weeks following the surgery. If you want breast lifting and implant, you should use breast band with sports bra for 6 weeks after the surgery. Sports bra supports your breast from downside and breast band supports them from the upside. By this way breast tissue and implant will be moving together.

Round implant can give a more natural result with upper pole fullness for your case. You have not enough breast tissue, because of this I recommend dual plane technique for you. In this technique upper part of implant is covered by chest muscle and breast tissue, lower part of the implant is covered by breast tissue. Asymmetrical implant should be used for your surgery. With this combination you can get good result. This operation takes 1.5 hours and can be done under the general anesthesia. You should stay one night at the hospital to get rid of the anesthetic drugs affect. After surgery you should use sports bra and breast band for 6 weeks. Two weeks after the surgery you can start sports but you should wait for 6 weeks to use your arm strongly.

For your case I recommend; decreasing of the bone wideness with osteotomy ( your nasal bone can be approximated to each other) , height of your nose can be increased with cartilage graft, tip part of your nose can be reshaped to get triangular, more defined, feminine nose tip shape.
