Your double chin in due to a number of components. There is likely still some residual fat in the area but there is definitely redundant skin. Facetite would be an option in addition to some liposuction performed at the same time. This would improve the contour of the double chin. However, it will not be as effective as a surgical necklift. I can't tell completely from the photos but there may be some laxity of the muscle layer as well and facetite and liposuction does not address this. The gold standard would be a necklift, liposuction, and possible platysmaplasty (can't tell from photos completely), but I do think you can expect some improvement with a less invasive facetite and liposuction procedure.

Whether a chin implant or fat transfer can be performed in the office will depend upon your surgeon. That being said, in my practice they can definitely both be done in my office surgical suite under local anesthetic. Most facial surgeries that don't require extensive amount of time or deep dissection can be performed in the office as long as the practice has a dedicated room with adequate lighting, room set-up and supplies, and staff experience. It depends also upon the patient's comfort level with doing it under local and oral valium, as well as the surgeon's comfort level. Good luck.

Photos would be helpful but in general there are is no absolute maximum amount of filler that can be injected to the under eye area. It is a case by case decision. However, I would be very cautious about doing additional filler to this area. I generally stop at one syringe total per side, and this is usually done over multiple sessions. There are likely some instances when more than one syringe per side can and should be used. However, in my experience, when you start using too much filler in this area you risk issues with weird swelling and puffiness that looks unnatural. In these patients I often perform lower blepharoplasty with fat repositioning, and then after surgery if they even need filler they typically need much less. Hope this helps.

Thanks for the question. Based upon your photos I do not see eyelid ptosis. As you correctly pointed out, the main difference between the two eyes is that the eye on the right side of the photos appears to have less volume in the sub-brow area and also the area known as the superior sulcus. In addition, the brow on that side is higher which can contribute to that difference in volume. You may be able to adjust brow position using botox. You may also be a candidate for filler injection carefully into the area underneath the brow and upper eyelid to improve the difference in volume. I would not recommend ptosis surgery. The main difference I see between the two eyes is that the eye on the right side of the photo (I believe it's our left eye although I am never entirely sure due to how selfies work) appears to have less overall volume in the sub-brow fat. This leads to a slightly more hollowed out appearance. This can often be corrected with filler injection to the upper eyelid/sub-brow area. I would suggest seeing an oculoplastic surgeon with experience in this area, or another experienced physician injector who does filler in the upper eyelid. Not every injector is comfortable with this specific procedure. Best of luck!

There can be many causes for eye asymmetry. Simple reasons include differences in the amount of skin and how skin folds in the eyelids. Sometimes eyes sit at different locations in the eye socket which can cause asymmetry. Even differences in the amount of glasses prescription can cause a difference (ie one eye very near sighted compared to the other). Very commonly it is due to differences in how muscles contract around the eyes. Some people naturally have more strength or muscle contraction on one side compared to the other. In order to fully determine your cause you would need a complete and detailed exam to rule out other causes. That being said, based on the photos you have sent some of the asymmetry could likely be easily corrected with a few units of botox injected into the your right lower eyelid as it appears the right lower eyelid muscle contracts more and therefore elevates more whenever you smile. I see this all o the time and it can be easily corrected with precise and careful botox injections. Hope this helps.

It depends upon which area of the eyelid you are talking about. I wouldn't inject filler in the area of your epicanthal fold (right by your medial canthus) because this could cause an unnatural appearance of the medial canthus and normal fold in the eyelid. Potentially a small amount could be injected more laterally to address some early volume loss but this would require very careful and precise injection technique.

Without a good photograph I cannot answer the question specifically. However, generally speaking the approach to over done eyelid surgery focuses on two main things. First, the degree of deficiency of skin and second the degree of deficiency of fat. If too much fat was removed there may be ways to reposition fat that may be left and available to simply move from one spot to the other. Secondarily, fat grafts can be placed back into the eyelid either through fat transfer or something called a dermis fat graft. Filler can also be used sometimes to fill in areas of hollowing. If too much skin was removed sometimes restoring some volume with the above mentioned intervention may help the appearance of a skin deficiency but if it truly is a skin issue sometimes a skin graft may be needed. I usually only recommend this if you have a hard time closing your eye and are experiencing severe dryness because aesthetically speaking, a skin graft can look bad at times. Best of luck.

Thank you for your question. Based upon your photos and anatomy, I believe a lower blepharoplasty is the best option for a natural, long lasting result. Filler is often considered to hide lower lid bags and could be performed but in patients, especially men, with prominent fat like you have a lower blepharoplasty gives the best and most natural result. A key to success is the technique performed and I prefer the surgery to be done from the inside of the eyelid and to resposition most (if not all) of the fat into the cheek/tear trough as opposed to simply removing all of the fat. Can also consider a pinch of skin or laser resurfacing for the skin. Best of luck.

By seeing an experienced and qualified eyelid surgeon an upper blepharoplasty can absolutely improve the appearance and heaviness of your eyelids without causing you to lose your naturally more full upper eyelid. A conservative blepharoplasty removing usually only skin would give you an excellent result that is natural yet refreshed.

Love my Skin Pinch & Canthoplasty Results

Dr.Straka did exactly what i wanted him to do and I am very happy with my results. I was really worried about retraction and this procedure possibly being a mistake because of some of the things I read and because I didn’t have a lot of skin under my eyes but I just decided to do it anyways and I’m glad I did. I am almost 70 and I did not want to continue on this aging path with my eyes. My main advice - only use an Oculoplastic Surgeon like Dr. Straka for your eyes. I had a horrible experience with a Plastic Surgeon who wanted to put $7000 of fillers in my eyes as a solution. For 10 years I have used botox intermittently to help refresh my heavy eyelids. Now at 50, the botox could no longer do the heavy lifting of the redundant, excess skin. I visited/interviewed multiple providers in Wisconsin but ultimately wanted a specialist with many photos of successful outcomes. I did not want to travel to the east or west coast and was happy to find an experienced provider in the midwest, less than 8 hours away. We did a pre-consult via Zoom in October. I went by myself in January and used La Perle Concierge for 8 hours of post-op care. (Mainly to get me to the hotel and check on me the following day) The Columbus Eye Surgery Center is fantastic as is Dr. Straka and his team. I stayed 7 days at a local hotel that offered both breakfast and dinner and it was less than $100/day..like a cruise! Dr. Straka performed upper/lower eye lid bleph with fat repositioning toward my tear troughs as well as fat grafting from the abdomen. I have a smoother contour of my lower eyelids now. I took my 1989 grad photo (big hair and all) so he knew that in general I have always had heavier upper eyelids but just wanted to remove the lax skin. I still look like me only more refreshed. My eyes are much less tired at the end of the day too. Although recommended I did not do the TRL contour laser for the undereye wrinkles. We were spending time in Florida and I knew this would prolong healing. I am so happy with everything and would certainly recommend Dr. Straka as he is an expert in his field of oculoplastic surgery. Lower bleph, endoscopic brow lift, fat transfer & laser resurfacing

This is my second attempt to improve my eye area. In 2015, I had upper and lower bleph w/TCA peel with a different doctor and the results were underwhelming. I was afraid of having any other work done right away for fear it would look worse. As my wrinkles and hollowness under my eyes were getting worse over time, I started researching surgeons. I consulted with 4 surgeons from Cincinnati to Columbus and was referred to Dr. Straka by one of those surgeons. After visiting his website, social media pages and having a consultation, I was sold. We agreed to lower bleph, endoscopic brow lift, fat transfer, and laser resurfacing for the best possible outcome. I had local anesthesia and never felt any pain. I don't remember a lot of the surgery, thankfully. To try to summarize the past 6 weeks: Recovery - (depended on my husband for virtually everything for the first 3 days). Had to sleep upright for a week, could not bend forward for 3 or 4 days; but the worst part or me was about day 3 & 4 when the goop in my eyes caused my lids to stick together making it hard to open my eyes. It usually happened after a period of sleep and it really freaked me out. My husband texted with Dr Straka and he reassured us this was normal and not to be alarmed, and suggested a warm compress. That helped a lot. I had several staples in my head that needed removed at day 6. I was terrified that it would be painful but I honestly did not feel a thing when he removed them. All that anxiety for nothing! Haha. Approaching 7 weeks post, I see incredible improvement overall. When my face is at "rest" I don't have hollows anymore.With a small smile, the area is virtually wrinkle-free. However, when I smile big, there are still some stubborn wrinkles that refuse to go away. I did express my feelings about that and Dr Straka humbly acknowledged that they may never go away, and perhaps a small amount of Botox might help. I am still thinking/considering that. Recovering from the laser was by far the hardest part. But after healing, it was so worth it. I can see a big difference and I am ecstatic. Dr Straka is masterful, thoughtful and attentive. My only regret is that I didn't find him sooner.

Malar edema resolved and provided a more youthful look to my mid face

In 2016 I developed sudden malar edema post neurotoxin injections around my eyes. I tried many options to end the edema, and none worked permanently. I saw Dr. Straka's IG showing a repair of a situation very similar to mine and went for a consult. He suggested a comprehensive approach of lower bleph with fat transfer, a 3-treatment serious of morpheus8 for skin tightening, TRL around my eyes to improve skin quality and treat crows feet and a soof lift to provide a more youthful look to my mid face. He was very clear that malar edema is incredibly difficult to treat and that he couldn't promise complete resolution, but that I would definitely see an improvement. I decided to go for it, and now, 4 months post-op, could not be happier with the results. I would highly recommend Dr. Straka and his team if you are interested in this kind of procedure.

Facial plastic surgery can be a very overwhelming process from start to finish. To searching and researching the right doctor, to the consolation, surgery, follow-up, etc. Dr. Straka from start to finish was amazing! Listened to goals, explained process, patient care, follow-up, and the most important - RESULTS! We had a face / neck lift, upper and lower eyelids, lip lift, fat transfer, TRL, filler, and Botox, all amazing results and wouldn’t recommend anyone else. I have had several procedures done from eyelid surgery, to laser treatments and finally, a facelift. Dr. Straka is not only extraordinarily skilled and talented, he made it a point to be clear about realistic expectations and outcomes for each procedure. We were always on the same page because I knew that he was being completely honest and forthright with me. Beyond his professional expertise, Dr. Strakaq looked at my face as if he were an artist and helped sugest what treatments, surgery or procedure would produce the best results from his "palette". Dr. Straka is passionate about his work and that translates into a shared sense of pride with his patients when we both can look back and see the amazing results. I also felt that Dr. Straka looked at me as a whole person - not just cosmetically -he wanted to know my motivation and my place in life for what would work best for my age. Therefore, he could better understand and ascertain how to achieve "our" goals. When people discuss plastic surgery and they evaluate how they see what others have done, they generally say one of two things: a. "It looks like they've had work done" (BAD) b. "You can't even tell they've had work done" (GOOD) There's a certain irony in these comment when it comes to Dr. Straka because it is evident that he doesn't view his profession as "work." It's more of his mission to provide the very best that he can for his patients. He was committed to treating me as an individual - (no matter how minor my questions or concerns may have seemed.) Time spent with him in the office, or the surgery center, aftercare and even holidays and weekends never seemed like an imposition. I received responses promptly from him his office team and everyone involved in my care. I would be remiss if I did not mention his staff that that helped me through every procedure- Cristina, Kathleen and the nurses in the surgery center. They all treated me as if I were their most important patient and I never felt like a number on a surgery schedule.
