There are many factors that influence your final breast shape after breast augmentation.  Your age, height, weight, chest wall size, breast width, where your breast tissue is positioned on your chest wall, how much or little breast tissue you have to cover the implant, the thickness of your subcutaneous fat layer, any history of pregnancy/breast feeding, and nipple position  All of these can influence breast shape and position of the implant after augmentation and your final outcome.  The first picture looks like the final breast shape is due mostly to the round implant and the implant is making up most of the breast volume.  There appears to be very little breast tissue or fat. She most likely is in her twenties and never had a pregnancy. The second picture shows a woman with some slight ptosis (natural sagging) to the breast and a larger implant.  Her pre-op photos most likely will show more initial breast tissue. The third picture is in a bathing suit and a little harder to compare but likely her nice shape is due to more breast tissue initially.  Meeting with an experienced plastic surgeon can help you achieve the result you want but your natural body/breast shape and size will be the most important factor influencing your final outcome.

You'll have some moderate discomfort for the first few days which might require narcotic pain meds and Valium for muscle spasms.  You can switch to Ibuprofen beginning 24 hours after the procedure. Most people can drive by 4-5 days after and return to work or school within 7-10 days. Once the pain has subsided you can do light activities. No heavy lifting or strenuous exercises for 3 weeks. After 3 weeks begin very slowly doing modified workouts and after 4 weeks full workouts. The swelling will be the greatest in the first 2 weeks but can last for several months.  As your muscle relaxes and the skin accommodates this new volume, the breasts will soften and appear more natural.  Sensation may change after the procedure but typically returns to normal within 3 weeks, but you might be hypersensitive for a few weeks.

In general it's important to be active after a long surgery like a mommy makeover.  Risk of blood clots in the legs increases if you have a tummy tuck and add additional procedures.  Light activity is started immediately after and then gradually increased.  For more strenuous activity and training, I would wait until 3-4 weeks before beginning and gradually increase over 2 weeks.  You should be full go and busting your butt by 6 weeks.  That should be plenty of time for getting up to speed with the training! Recovery time varies from person to person.  I tell my patients to plan on being off work for 10-14 days.  It's important to be active around the house immediately following the surgery.  This helps keep your circulation moving and helps prevent blood clots from forming in your legs.  After the first week you'll start feeling like you got this, and your recovery will get easier!  I prefer no strenuous activity or heavy lifting until after 3-4 weeks, but start any activity slowly and gradually increase over a week or two.  You should be full go by 4-6 weeks.  You will have some swelling that will come and go based on your activity, but may stick around for a few months.  Good luck! and realize it will not be as bad as your imagining it will be!

There are many factors that contribute to the final size and appearance of breasts after breast implant placement.  Your height, weight, chest circumference and shape, volume of breast tissue, and degree of ptosis can all contribute to the final outcome. Certain bra manufacturer's bras are smaller than the stated size.  Your current bra makes your actual breast size more like a B cup.  Adding 350 cc to a B cup breast will probably get you to a D cup.  Though it is difficult to estimate final breast size without an exam to determine your current breast volume. Trying on sizing implants in a non-padded bra may help with your final choice.  This can be accomplished at home with a knee high nylon stocking filled with a measured amount of dry rice or oatmeal (in cc's), shape it into a round "implant." Implant size is an important decision to make and experimenting with sizers will help you make the right choice.

The key to a natural youthful appearance to your breasts after a mastopexy/augmentation is to have the nipple at the most projecting point of the breast mound.  These pictures show some glandular (breast tissue) ptosis and areolar asymmetry. In my opinion, an implant alone may not fully fill out the excess loose skin and result in some persistent ptosis.  To minimize scarring and to get the nipple/areola symmetrical and in the correct position, a Benelli or peri-areolar lift may be the best option.  The drawback to this type of lift is a flattening of the breast. A thorough exam will reveal how much laxity your breasts have and will help determine the best type of lift you'll need.

No matter what type of breast lift you undergo there will be scars that will take time to heal.  Breast lifts can be around the nipple ("doughnut" or peri-areolar mastopexy), vertical ("lollipop" type scar), or a full lift ("anchor" type scar).  The scar healing process takes 12-18 months for the scar to fully mature and take on its final appearance. During this maturation process the scars will start off as a thin line, become red and slightly raised, and finally fade and flatten. There might be some irregular, firm, or un-evenness to the scars throughout this process but they should eventually smooth and flatten. The scars in these pictures are just starting to mature and have the typical uneven appearance and are slightly pink. This is a normal appearance at 8 weeks out.  There is some asymmetry to the areola (one is slightly larger than the other) this may not correct itself over time and may require a small revision that can typically be done in the office. There is also some "coning" appearance of the areola that can occur with the peri-areolar lift. This might remain long term.  When implants are placed under the muscle, it is normal for them to move with muscle contraction.  The "settling" of the implants occurs over a 3-6 month period as the skin and muscle relax and accommodate the recent increase in volume.  I would not be concerned with the scars and implant position at this time and allow things to settle.  Regular follow up with your plastic surgeon during the healing process is very important for re-assurance and guidance.

A Crescent mastopexy is performed by removing a crescent shaped piece of skin on the superior border of the areola. This is an attempt to raise the nipple when there is some mild ptosis (sagging) present.  However, this type of lift to the areola usually only distorts the nipple/areola complex and provides very little in terms of nipple re-positioning. A "doughnut shaped" mastopexy removes a circular piece of skin around the entire areola allowing a slight lift to the nipple without distorting it.  The "ideal" distance between the notch at the base of your neck and your nipple is approximately 19-21 cm. Based on your photo you have a more significant amount of breast and nipple ptosis, requiring more significant lift. You would likely require a vertical lift (lollipop shaped incision) or a full lift involving skin removal in the vertical and horizontal plane (Anchor shaped incision). An implant will provide the added volume you desire and will also improve the shape. If you don't have the more significant lift you will be disappointed with the shape of your breasts and they will still appear sagging. A complete exam that accurately assess's the amount of laxity in your skin is the best way to determine which lift is best for you.

The cost for a breast lift and implant procedure (or Mastopexy/Augmentation) depends on several factors including how much of a "lift" is required and what type of implant is used. A straight forward Breast augmentation in my practice is approximately $3500 for the surgeon's fee, $2100 for silicone implants or $1200 for saline implants. The cost of a combined breast lift/ breast implant procedure can range from $4000-5500 depending on how extensive the lift needs to be, plus the implant fee as above.  For any procedure there are also anesthesia and facility fees which range from $1400 (for 1 hour) to $2200 (for 3 hours) depending on how long it will take for the lift part of the procedure.  Total estimated costs including all fees for a breast lift with silicone implants is approximately $7500-9000. These costs do vary depending on your exam so only a private consultation and thorough exam and discussion with you plastic surgeon will give you the most accurate costs for this procedure.

Each woman's breasts have a unique size, shape, and appearance. When deciding on what type of implant to use or whether you need a lift or not it is important to have a thorough exam.  Factors such as height, weight, chest wall size, shoulder width, breast width, breast size, and amount of sagging are extremely important in deciding what will work best for you.  In my practice, several measurements are done to assess the dimensions of the breast and degree of sagging (ptosis) of the breasts. Many factors contribute to the appearance of your breasts: Age, Gravity, Pregnancies, breasts feeding, weight changes, and genetics.  It is normal to have volume loss after pregnancy with a lower nipple position, giving your breasts a "deflated" appearance.  Also most women have some asymmetry to their breasts in size, shape, or nipple size or position.  A breast augmentation can help restore the volume lost and give your breasts a fuller shape and appearance. Based on your photo you do have some degree of nipple ptosis (sagging) and areola size difference that can be corrected with a breast lift (or mastopexy). Most often women in my practice choose silicone implants for their augmentation because of the natural feel of the silicone implant but saline can also give you a nice result.  To determine the size of the implant to use, I encourage my patients to "try on" several sizes.  You can accomplish this by making your own set of implants. To make an implant: Take a knee high nylon stocking (not a plastic ziploc-that will give you square implants-not very attractive!), measure out in cc's or ml's with a measuring cup dry rice or oatmeal, start with 300 cc's and add or subtract by 25 cc's, twist tie the end and shape into a round "implant", place on top of your breast centered on the nipple in a non padded bra. Once you have your implants made, experiment with different sizes and different clothes (sweaters, dresses, tight shirts, etc) because they will look different depending on the thickness of the outfit. When you have the final size narrowed down, go back to your surgeons office and try on his/her implant sizers to finalize your choice.  I usually add about 25 cc to the final implant because it will look smaller underneath the muscle and breast tissue. I try not to choose the size for my patients but I am there for advice, opinion, and guidance.

Tummy tuck exceeded my expectations!!

I am four months post-op from a tummy tuck with muscle repair and liposuction, and I couldn’t be happier with my results. Dr. Fedele is truly an artist and an exceptionally skilled surgeon. The nurses were knowledgeable and kind throughout the process. Everyone was patient with my gazillion questions. After having children via C-sections and experiencing weight fluctuations, I was left with a sagging, rounded belly that wouldn’t go away with exercise and couldn’t be concealed, no matter what shapewear I tried. I did extensive research before choosing Dr. Fedele and was amazed by the transformations I saw in other patients—so I wanted to share my own experience. My scar, for a tummy tuck, is neat and will fade over time, but I’d much rather have it than what I had before. I didn’t realize just how much extra skin I had until it was removed—it even hung over my thighs! Also my lower area was sagged which I hadn’t noticed till I saw the results of the mons lift that the doctor includes with the procedure. Now, my stomach is completely flat and a six pack is peaking through! My only regret is not doing this sooner! I was also advised to add upper back liposuction to the procedure, and I’m so glad I did. A flat tummy wouldn’t have looked as good with back rolls, so the combination was definitely worth it. Thank you, Dr. Fedele, for giving me my confidence (and waistline) back!

Always knew I would want my breasts lifted after having babies. Dr. fedele worked on family members and came highly recommended. He had done my Botox and filler for years as well. So I realized I couldn't reach my goals without getting a tummy tuck, and end up getting a mommy makeover. I would have payed quadruple what it cost to get it. Worth it, worth it, worth it. I look and feel amazing. I'm a year post op, and it's really incredible how minimal my scarring has become. This man is an ARTIST. Love him and his staff. First class. If you are anywhere near CLE, you need a consult. I have friends who went to other docs in the area and paid less and it shows. If I never told anyone, I really don't think they could tell unless they saw the very light scars. I look totally normal, not fake, just like I never had kids! Like younger me! Do it! He and his staff were there for me every step of the way and so accommodating and flexible. Only con is the pain in the recovery. You will need Nannie's temporarily IMO if you have toddlers. You won't be able to care for them very well for about three weeks. Dr. Fedele and his team definitely do what they can to ease your pain, you are given great meds, but you will have abdominal pain as you heal of course. I just could not believe how long it hurt to get out of a car or get out on bed, or the impossibility of picking up a kicking toddler.

Only Regret Was Not Doing It Sooner

I spent most of my life being told I look tired and angry due to my heavily hooded eyelids. I’m only in my 30s so I was hesitant to turn to surgery, but sometimes genetics just aren’t in your favor and sometimes surgery is the best solution. I went to Dr. Fedele for a consultation, unsure whether I was best suited for blepharoplasty or an endoscopic brow lift to give my eyes a wider, more rested appearance. He determined a brow lift was the best answer and informed me of exactly what I could expect from the results. The process of scheduling the surgery was painless and the information packet provided was unbelievably thorough. The staff members at the office were great, but I was completely blown away by the staff at the surgery center. They were incredible and I really can’t stress that enough. They made me feel so comfortable, helped to calm my nerves, and were beyond attentive both before and after the surgery. I’ve never received better care from anywhere else. Back at home, Dr. Fedele called me on more than one occasion to check up on me to see how I was doing. I healed really well and love my results! I’m glad I put my trust in Dr. Fedele. Putting on makeup is so much easier and my outward appearance finally reflects how I feel. I’m so much more confident! The only regret I have is not getting the brow lift sooner!

Extraordinary Surgeon with Extraordinary Heart! Dr. Fedele in Cleveland, Ohio, performed abdominoplasty, liposuction, breast implant exchange & breast lift on me in June . I expected him to be one of the best Plastic Surgeons according to my research. He exceeded my expectations. His skills, results and heart are beyond spectacular. He is an extremely warm & caring Surgeon who listens. What came as a complete shock was his treatment of me while I was being prepped in the OR. I am 68 years old and I was terrified before my 6 hour surgery. Dr. Fedele walked up close to me, took my hand in his and kept constant contact with my eyes while he was talking to me softly. He told me not to worry, everything was going to be ok, he was going to take good care of me. I cannot tell you how this made a huge difference for me. It was incredibly comforting and reassuring and I felt 100% better. It calmed me down. I have had quite a few surgeries in my life. I never had a Surgeon or Doctor do this for me. Decided a few years ago after 70lb weight loss I would like to get a lift and implants. The staff here is wonderful! Dr. Fedele and his nurse Megan in particular are so caring and make you feel extremely comfortable every step of the way. I was so unsure of implant size that I went in a couple times to try on sizers and ask questions and they had no issues with this. They worked with me and made me feel confident with my decision. Day of surgery I was obviously nervous but Dr. Fedele made me feel at ease and told me how great I was going to look and that they would take great care of me. The whole surgical experience was so smooth and Dr. Fedele called me that evening to check on me, and gives you his direct line in case you have any issues or concerns! My results so far 2 weeks 3 days post op are so much better than I had anticipated! I worried myself sick that I had chosen the wrong size, the results wouldn't be what I wanted, I would hate the scars...but wow did Dr. Fedele prove me wrong! My lift incisions are almost non existent! He took great care in making me look AMAZING! I am so happy with my new and improved look. There Aren’t Enough Stars to Rate my Experience.

Before my surgery, so many people had told me I was wasting my money, that I didn’t need it and that I was beautiful the way I was. I didn’t see myself that way, and while I appreciated the kind words- I didn’t believe it. No matter who said it. No matter how many times I heard it. God himself could have told me I was beautiful and I would not have believed it. I needed to believe it for myself. Since my surgery, I’ve also had many people tell me that I did all of this for attention and validation from others, which, In some subconscious way- I’m sure a small part of this journey IS in fact my feelings of needing validation from others. But that’s a story for a different day- when the topic is childhood trauma. I did this for myself. I needed to be able to look in the mirror and love myself. I needed to be in a healthier mental state- just to keep living. Literally. My body has caused an immense amount of depression and self image issues for 13 years- and I was ready to feel better. REALLY ready. I weighed 196lbs in January, which is the heaviest I’ve EVER been- even during pregnancy- while Steve was only 10lbs heavier than me. Like what?! Even his T-shirts that used to be big on me we’re starting to get too small. I decided enough was enough. I am not comfortable with not even being able to fit into my man’s T-shirt if I want. I set myself a goal for 2021 to lose about 50 pounds and get a tummy tuck surgery. I had to start somewhere, so I started researching plastic surgeons from here to Miami to Mexico. I finally found one that I felt very confident in just from looking at his online portfolio. Lucky for me- a nurse that I used to work with (who is a great nurse) works for him now so I had double the confidence and comfort of actually knowing who would be taking care of me. I was actually already established at a different plastic surgeon and decided to leave. I wanted the best of the best when it came to altering my body. ??? I didn’t meet my goal weight by the time of my surgery, which was disappointing, but the motivation to maintain and continue to lose weight solidified when I first got a glimpse of the new me- the day after surgery. As of today I weigh 159.2lbs. I have lost 36.8lbs. Let me tell you- that’s just what I got. The BEST of the best. ?What Dr. Fedele was able to do for me- I could never repay him for. I honestly feel like what I paid doesn’t cover the incredible results I have, let alone the drastic improvement in my quality of life. Just six weeks ago I dreaded getting up in the morning and looking at myself. Don’t even talk about clothes shopping back then- or taking a photo that wasn’t from my neck up. I allowed my self image to dictate what I did in life. I was extremely picky about my clothing and learned how to dress to hide my unflattering figure- which is probably why some said I was wasting my money and didn’t need the procedures. To think that I allowed that to go on for so long, causing so much mental anguish, is ridiculous. I know I’m also not the only one who has struggled with this.
