A: Based on your photos and post, I agree that you have a superficial thrombosed vein in the area and I think its due to the filler rather than a nicked vein. The good news is that it should resolve on its own and I do not believe it poses any immediate risk or threat to you. You can always try to dissolve your filler and see if that helps
A: Based on your photo, I would not recommend placing any filler in the area under your lip because it will accentuate those "bumps" that you have. You may benefit from some chin filler but it's hard to tell based on the one photo you provided
A: Yes you are a good candidate for lower blepharoplasty. The technique I would use in your case would be transconjunctival (hidden incision) with lower lid fat repositioning or fat grafting.
A: You are a good candidate for rhinoplasty. The nasal tip can be narrowed and more defined with rhinoplasty. Your profile is very nice but you have a slight dorsal hump which can be reduced and make it look even better.
A: In my professional opinion you need to give the tissues time to heal. The skin and tissues to your nose have been through alot of trauma between multiple rhinoplasties and now a vascular occlusion. I would be very hesitant to do anything else to the tip of your nose especially with an energy based modality. Just give it time and try to cover up with make-up as best you can.
