Would an Axillary Breast Removal procedure also help lift the breast?

Thank you for your question. Unfortunately, an axillary breast procedure (either via direct excision or liposuction) will not also help to lift the breast and should be thought of as a separate surgical procedure. Oftentimes axillary breast tissue is a concern to breast surgery patients and can be addressed at the same time as a breast lift or breast reduction for example. I was supposed to stop smoking for weeks before my BA. It is now in five days and I just stopped smoking. Will I still be OK?

Thank you for your question. I would strongly recommend that you discuss your history of smoking and the fact that you only stopped smoking very recently with your board certified plastic surgeon. Smoking and use of nicotine containing products constricts the small blood vessels and can increase your risk for surgical complications - these complications can include poor healing, scarring, wound formation, etc. Each surgeon has their own specific recommendations when it comes to smoking and nicotine use/stoppage pre-operatively, so your surgeon will be best qualified to advise you. Am I good candidate for an extended tummy tuck or a body lift? (Photo)

Hi and thank you for your question.  If you are looking/planning on loosing some weight, I would encourage you to hold off on undergoing any elective cosmetic procedure until you are at a stable and maintainable goal weight.  Without your height it is impossible to calculate your BMI, but it is always advisable that patients have a BMI of 30 or less to reduce your risk of complications post-operatively.  

From your photos you do appear to be a good candidate for a bilateral breast lift and tummy tuck +/- liposuction of your thighs.  From your side profile photos your abdomen is barreled in shape which is caused by intra-abdominal or visceral fat.  While this can be improved upon with a tummy tuck, it is important for you to be aware that without pre-surgery weight loss your abdomen will still maintain this barreled shape after surgery.  

Since you are considering some additional weight loss before surgery, starting your search via virtual consultations with board certified plastic surgeons is a fantastic idea.  These virtual consultations can help you get a feel for what your surgical options are and which surgeon you are most comfortable with.  While it is always important to seek the opinion of board certified plastic surgeons regardless of where their practice is located, I would encourage you to schedule a consult (or two) with a local board certified plastic surgeon and to consider the additional risks of traveling in this new post-Covid19 era.  I hope that this helps and I wish you all the best!

Do I have tuberous breasts or just small breasts? (Photo)

Thank you for your question.  From your photos you appear to have constricted breasts (I would not classify them as tuberous breasts) which could be addressed via a breast augmentation with small breast implant.  I hope that this helps!

Will I need a lift also when I replace my implants? I got 440cc silicone implants 6 years ago under the muscle. (photos)

Hi and thank you for your question.  Looking at your photos it does appear as though you would likely require a breast lift to improve your overall breast shape.  You can definitely exchange your implants if you are looking to go bigger, but a lift alone is also a possibility if you are happy with your current size.  My concern though would be the pain that you are experiencing as it may be a sign of a capsular contracture.  

I would recommend scheduling a consultation with a board certified plastic surgeon for an in person evaluation.  Your consultation will be the perfect opportunity to review your concerns and goals in greater detail and to have your surgeon of choice evaluate you to see what may be contributing to the pain you are feeling.  I hope this helps!

What is recommended for my stomach and back? Is a tummy tuck my only solution? (photo)

Hi and thank you for your question.  After evaluating the photo that you shared I believe that you would be best suited for a tummy tuck with some liposuction of your flanks.  Unfortunately, I don't think that smart liposuction was the best surgical option for you - especially if your youngest is only 7 months old currently.  Generally speaking, I prefer to wait at least 6 months before performing a body contouring procedure post-pregnancy/delivery.  Waiting this period of time allows for your body to normalize and heal post-baby.  While any 'laser assisted' liposuction procedure can enhance your liposuction results, there is a limit to how much skin retraction patients can (and should) expect.  

I encourage you to schedule a consult (or two) with a board certified plastic surgeon who is well-experienced in body contouring procedures.  Please remember that a board certified cosmetic surgeon is not the same thing as a board certified plastic surgeon!  I hope that this helps and I wish you all the best.

Under muscle silicone implant removal after 24 years - Where is the incision?

Thank you for your question.  The incision to be utilized for your explant will be determined by the preferences of your surgeon.  Most board certified plastic surgeons avoid using the axillary incision for both implants and explants, and if you are proceeding with a breast lift it would make sense to utilize those incisions so as to not increase the amount of scarring.  You might also want to discuss the possibility of having a breast lift at the same time as your explant - if possible.  I hope that this helps!

How will I know when my silicone implants need to be taken out or redone? I’ve had them for 8 yrs and never had an MRI

Hi and thank you for your question.  If you have silicone gel implants (FDA approved and implanted within the US by a board certified plastic surgeon), there is no need to replace them unless you are having an issue with them or would like to go bigger/smaller.  I believe that the conversation about replacing breast implants stemmed from the warranties offered by the various implant manufacturers.

If you are concerned that there might be an issue or compromise of your implants it would be prudent for you to schedule a follow up appointment with your board certified plastic surgeon.  When it comes to silicone gel implants, the only way to identify an issue (for example, an implant rupture) is upon imaging - and a MRI is the best option.  I hope that this helps!

I'm currently 34E. What size would I be if 400 grams are removed from each breast? Dr thinks they're currently 800 grams (Photo)

Hi and thank you for your question.  I perform many breast reductions and work with insurance companies often and it is always a difficult discussion with patients when it comes to insurance requirements for amount of tissue removed to obtain medical necessity coverage.  It is difficult to say with any certainty what cup size you would be after a 400g removal bilaterally (especially not having evaluated you in person), but it is safe to say that you would be significantly smaller.  I would encourage you to follow up with your board certified plastic surgeon to discuss your concerns in detail and to see if there is any possibility of conducting a peer to peer review if your surgeon thinks removing the insurance required amount of tissue might leave you with a disproportionate result.  I hope that this helps and I wish you all the best.

Do you have to have a tummy tuck to get a BBL?

Thank you for your question.  No, a tummy tuck is not required to get a BBL.  Your surgeon might simply be recommending a tummy tuck if there are concerns about your skin quality and current degree of skin laxity.  

When it comes to a BBL, the degree to which you can enhance the buttocks is limited by the amount of fat available for harvest.  A tummy tuck at the time of a BBL is often not recommended as you are limited with how much liposuction you can perform on the abdomen and flanks at the time of a tummy tuck and you would therefore be limiting the amount of fat available for use during the BBL.  If you are OK with the understanding that you might have some additional loose or redundant skin of the abdomen with liposuction alone OR if you understand that you may want or require a tummy tuck in the future, I would encourage you to discuss having a BBL first with your board certified plastic surgeon.

I am now 1 week post op

I recently underwent a breast lift with augmentation under the care of Dr Blaine. After researching other doctors on Long Island , going to consultations and reading the reviews on this site I decided to consult with Dr Blaine. Her office is not only esthetically beautiful but her female staff is unbelievably kind and comforting. From the receptionist to her nurses to the office manager everyone made me feel not only welcomed but comfortable. During my consult Dr Blaine never once rushed me and answered all of my questions with not only knowledge but consideration for my concerns. I scheduled my surgery 3 weeks later and her staff was so supportive before and after my procedure . Tati actually rubbed my hand before I went under and told me everything would be fine. The nurse called me the evening after my surgery and Dr Blaine followed you with me that evening at 10pm!!!! She was genuinely concerned about my well being and wanted to make sure I was ok. I am schedule for my BBL for June 2, 2020. I am excited and nervous. I worked out for two year straight.. and for good results. But I want to remove stubborn areas and enhance the areas to make my body proportion. I currently weigh 143.6 (3/2/20). My height is 5"3. Updated on 4 Mar 2020: Here are some wish pics.. I am looking for a natural look. Since I had my breast augmentation done 2019, I want my body to look proportion and I will to work out and maintain this figure. I am so excited for June to come. I have to start making arrangements in getting lymphatic massages afterwards. Updated on 28 Oct 2020: Due to Covid, I reschedule and focused on working out. Updated on 11 Jan 2021: I went got my 360 Lipo and BBl done. She Lipo from my about 900ccs. And injected 700 into my buttocks. I weigh 143 and I am 5"3 in height. The day of my surgery I felt no pain afterwards. The 1st post op, felt no pain. The next day, once the anesthesia wore off then I felt the pain. I've been taking my antibiotic, my regular vitamins and Tylenol 600 milligrams. I have not taken any narcotic( not a fan of it) but the 600 is working fine for me.

Finally Doing It!! Tummy Tuck for my 50th Birthday!

My surgery is scheduled for the January 29th 2020. Really excited. I have been wanting a tummy tuck for many years and now I am doing it. I also added Lipo for my back. I am adding photos. Will keep you you posted with after pictures in February. Happy new Year! Updated on 2 Jul 2020: I had my surgery June 30th I am so glad that I finally did it. Day 3 and I feel ok, sore but good. I will take some pictures soon. It was helpful for me to see other's experiences and photos so I will try to share mine. Updated on 9 Jul 2020: Dr Blaine and her staff are the best! I originally was going to have my surgery in January, I have diabetes and was not on medication so we started medication and postponed January surgery. Treatment for that my Diabetes is a success now and my A1C is under 6 now so I am lucky that I caught it soon. It runs in my family. Then the pandemic happened and more delays. Had my procedure June 30th I am 9 days in and feel great.I can see how it is going to look. Very Happy. I get my left drain out tomorrow. My right one was removed yesterday.
