You can still do Invisalign if you have a crown. However, if you have an implant, that tooth cannot be moved at all.  You would only be able to move the other teeth around the implant.  I would recommend waiting to have any implants placed until after Invisalign/orthodontic treatment is complete.  Your case looks like it would take between 1-1.5 years to correct.

It is difficult to say without seeing complete photos of your mouth, but if your main concern is that the front teeth protrude, then you will see the most change if you have teeth extracted.

It is necessary if you want to keep your teeth perfectly straight.  Depending on how your teeth looked before braces, teeth always try to return to their original positions especially in the first 6 months

Set up fee; is this legal?

Did the office take any records on you such as x-rays or photos?  If so, that may be what the charge is for.  If you do have to pay for those records, you can ask for copies of them to being to your new orthodontist.

yes, you can transfer your case, but I would recommend seeking treatment from another orthodontic specialist, not a general dentist. Orthodontists go to extra years of school to learn every detail on how to move and position teeth.  If you are seeing a general dentist and he/she is struggling with your case because it is challenging, it may be wise to seek treatment from an orthodontist because they have a lot of experience treating the most difficult cases.

4 months sounds a bit short looking at the photos.  Most cases require more than 6 months to obtain alignment and a proper bite.  Also retainers are generally recommended for a lifetime use or else you risk your teeth shifting again
