5 out of 5 stars Breast Augmentation
I seem to remember as a teenager having proportional breasts for my 5'6'', 135 pound frame, but after losing some weight in college, they just disappeared into roughly A-cups. I was pretty half-hearted when initially looking into breast-augmentation, but the more I looked into it, the more common I found it really is. With the "Hollywood-only" myth removed from my perspective, I got really...
5 out of 5 stars Breast Augmentation
I did more than six months of research, appointments, interviews and reading before making my final decision to go with Dr. Tholen. He was my first appointment because I suspected he would likely be the surgeon I would select after following his profile for several months on Real Self. I did interview three other reputable and popular surgeons in the Twin Cities, but I knew that Dr. Tholen was the right choice based on his years of experience, bedside manner and professionalism. I should preface this review by saying that my husband was adamantly opposed to augmentation and loved me just the way I was. He joined me during the appointment with Dr. Tholen, and I could tell that Dr. Tholen's sense of humor, candor and surgical expertise softened my husband's attitude and set a benchmark for appointments going forward. Dr. Tholen explained to both of us how nursing had changed my tissues and said that I was bordering on needing a lift, but he believe he could give me a could result without one if a sizable implant was chosen. He did warn me about the possibility of how a "borderline" case like my own might go, but said that if I were his family member, he wouldn't do it initially. I appreciated that he didn't scare me into an extra surgery at an additional cost, but at the same time was candid about the reality. Most importantly, Dr. Tholen was reassuring about my feelings and the emotional desire for the surgery and that he would work with me to achieve my goals. The staff was friendly and professional and took their time to walk me through all the appropriate information, but also were respectful of my time and were on time in the fitting appointments prior to the surgery date. On the day of the surgery everything was running on time, possibly ahead of schedule. I met with the nurse, anesthetist and was photographed. All were kind and did a great job of explaining the process to me and my husband. I once again met with Dr. Tholen and he was every bit as friendly, funny and informative as the first time we spoke, and we both easily agreed on the size, and then the next thing I knew I was waking up from surgery. The size appears to be everything I hoped for, and they are taking shape very nicely. At my follow-up appointment I arrived early, and they squeezed me in right away. I had some stomach issues from the meds in the days following, and my calls were immediately taken and addressed by the staff, and I could hear Dr. Tholen's voice providing instruction -- I feel like they have been the most accessible medical team I've ever had, and it is so reassuring, and it was helpful for my family who cared for me the days following. If you're trying to decide on a plastic surgeon in the Twin Cities, here are some things to consider: Expertise: You will find only a couple of surgeons who have as much experience as Dr. Tholen in the Twin Cities. Daily/weekly practice of surgical techniques counts for a lot, and this isn't like getting your hair colored, you don't want to roll the dice with someone who doesn't have a lot of patients. Information Sharing: Read Dr. Tholen's 3,000+ answers to people on Real Self, it will give you a good sense of how much he loves plastic surgery and wants to help educate patients. His answers are scientific with real-world context and can be understood in layman's terms. Professionalism: He respects the feelings and wishes of his patients and listens to their needs. Some doctors are happy to tell you what would look good on you. Dr. Tholen asked what my goals were and helped me understand if they were realistic. I also think it should be noted that Dr. Tholen works with transgender patients. While I might not be transgendered, I want a surgeon who treats every community with dignity and I think this is a good sign of being open-minded. Thank you Dr. Tholen!

Dr Tholen is the best! His work is amazing! I feel like my breasts are perfect, I could not have imagine they would look so great! Very happy with my tummy and lipo too, I cannot believe how flat and smooth my midsection is. I healed quickly and that is due to the great care I received from Dr Tholen and his staff! I highly recommend Dr. Tholen at Minneapolis Plastic Surgery!

I have had 2 procedures with Dr. Thollen so far and going to have more. He is a real Master. State-of-art body tuning. My first encounter with Dr. Thollen happened due to breast augmentation surgery. I needed it due to 15 lbs weight loss which resulted into sagging breast. I was looking for a doctor and my esthetician at MediSpa advised me to get consultation at Minneapolis Plastic surgery. ...

5 out of 5 stars Adam's Apple Reduction
Dr Tholen is simply the best doctor that I could have gone to for this type of surgery. His knowledge and familiarity with the needs of a trans woman is such a wonderful thing for the trans community. His attention to detail in every aspect from consultation to actual surgery is quite amazing to experience. He really cares about his patient and will go the extra mile to get you the best result possible. In my particular procedure he said that he had to go an extra hour to get everything just right. He encountered tougher bone and had to chip away a piece at a time instead of just shaving off cartilage from the tracheal area. He eventually got it just right and even did a bit of liposuction on my neck to feminize it even more. I was astonished and amazed at how incredible my neck does look. I could not be happier or more content! If you need any plastic surgery or anything else, look no further but to Dr. T! He is the best out there!
5 out of 5 stars Body Lift
A thorough research process of the procedure led me to conclude that I was looking for a practice that included: 1. a board certified plastic surgeon with extensive experience in lower body lifts. 2. an office staff that exemplified a team approach to delivering excellent care in all aspects of the process. 3. the opportunity to have my surgery in a free standing outpatient operating facility. (lower risk of infections; surgeon and support staff are well orchestrated, efficient team in familiar setting). By choosing Dr T, all of the above qualities were quickly evident and my expectations exceeded. In addition, I found that I was treated with the utmost respect and dignity. Each and every one of my questions was answered completely, patiently and enthusiastically, both by Dr Tholen and his knowledgeable staff. There was a prevalent attitude that this group of professionals was proud of their quality care, and loved their jobs. Happy workers make happy patients! I felt totally prepared for what the operation and recovery would be like. Dr T delivers care that is based on sound evidence leading to predictable consistent results - you will not find any gimmicky gadgets or procedures that are experimental or merely for marketing purposes. I have quite a bit of experience with behind the scenes doctors offices, and this one is the real deal. The other practices with whom I consulted all had some good features, but Dr T's practice had the complete package.

If there was a category beyond excellent Dr Tholen and his staff would have earned that rating. No question or concern was left unanswered and their confidence and professionalism relieved any anxiety I had.

As an aesthetic plastic surgeon for nearly 30 years, I am continually striving to find innovative ways to solve patients' aesthetic concerns, to correct or improve suboptimal results from other surgeons (as well as a few of my own), and how to best avoid them in the first-time patient.

Many medical products and pharmaceuticals are used "off-label." Botox is one I use frequently! Botox was originally FDA-approved ONLY for the treatment of glabellar frown lines (between the brows). Recently (2013), use in the "crow's feet" areas around the eyes was FDA-approved. But prior to that time, I would bet you that every one of my colleagues saying you can't have silicone implants until you are 22 injected countless of their patients with Botox for crow's feet! And forehead...
First of all, rupture is nearly impossible with the latest generation of cohesive silicone gel implants--and that is true for mammography as well as most any kind of "usual" or minor breast trauma. They cannot leak, and I believe that "periodic MRI scans" to check silicone implants for "leak" is both nonsensical and unnecessary. Rupture would require an exceedingly large force, and while mammography involves firm pressure on a woman's breasts, this is insufficient to damage implants. I have...
Drains are used to remove blood (or later, serum), and in some areas, such as tummy tucks, where there is a large potential space for blood or fluid to collect, drains can promote tissue adherence and healing. This is one potential reason to use breast drains--textured implants that must adhere to tissue to prevent inadvertent rotation or malposition. But even here that may not be optimal--acceptable, but not optimal! The problem is that drains allow fluid or blood OUT of a surgical...
Lots of surgeons tell patients they are allergic to Vicryl (sutures). I believe they are wrong. Vicryl sutures (made by Ethicon) are made of polyglactin 910, which is a copolymer of 90%glycolide (polyglycolic acid) and 10% L-lactide.These are braided, absorbable sutures that retain 75% of their original strength at 14 days, and 25% at 28 days. Vicryl Plus sutures have an added coating of triclosan, a broad-spectrum antimicrobial. Both absorb by hydrolysis, which...
Go to Articles for Part 1 of Dr. Tholen's Guide to Breast Augmentation. This begins Part 2: Implants and Breast Cancer; Mammography Numerous scientific studies have shown that augmentation mammoplasty does not increase the risk of developing breast cancer. Even without a family history of breast cancer, a woman has an 11% risk of developing a breast cancer in her lifetime. If there is a family history of breast cancer, these risks are higher. It is important to realize that while br...
