A: Hello, hope you are well. Thanks for posting your information and photos. I think you would be disappointed with liposuction alone. There is enough soft tissue laxity that tightening of the soft tissue envelope should be part of the approach. You could certainly accomplish a very good result with a facelift, but you would have to accept the scars of surgery, potential risks, and the investment of downtime, discomfort during recovery, etc. I would suggest a "middle path" procedure like FaceTite. This is a minimally-invasive procedure (no incisions) that will tighten the soft tissue envelope, slim/contour the lower face and neck, and reduce fullness from subcutaneous fat. It's ideal for a situation like yours. It is a smaller investment (time, money, discomfort, downtime, etc), and can be performed under local anesthesia. But, it's also a solution that's proportionate to the issue at hand. Good luck! Best, Dr. Tower
A: Hello, hope you are well. There are 3 procedures to consider: chin liposuction, NeckTite w/ liposuction, and a deep neck lift. These will give you good, better, and best results, respectively. Chin liposuction removes fat directly underneath the skin in the subcutaneous layer. We can combine that with NeckTite to tighten the soft tissue envelope around the jawline and further reduce the fat in that layer with radiofrequency energy. Finally, a deep neck lift allows even further contouring by removing the deep layer of fat that exists beneath the platysma muscle. A chin implant, CoolSculpting, and/or Kybella will likely result in disappointment. Especially since you still had the double at your lowest weight, it would be worthwhile to consider the neck lift option. Hope this helps! Best, Dr. Tower
A: Hello, hope you are well. Thanks for posting these photos, and a great question. Buccal fat removal will not reduce the width of your jaw and lower face. As you suggested, masseter Botox is a great way to start because it is minimally invasive and relatively low cost. However, it does not result in a permanent change. Jaw contouring is a straightforward procedure to permanently reduce the width of the jaw and create the desired result shown in your photos. This is done by shaving down some of the excess width of the jaw bone (especially near the angles of the mandible) from inside the mouth. Hope this helps! Best, Dr. Tower
A: So, yes, your nose is "strong". This can be adjusted. I'd give it 6 months to see how you do, but, it can be made smaller. But, what an improvement! You look so much more balanced and attractive! It can be hard to go from a large nose to a tiny nose, and sometimes it's easier to make the shape better. We can improve your nose, yes, if you want. But truly, you look so much nicer!
