The amount of stomach to remove for a sleeve gastrectomy has been extensively studied. Basically, surgeons want to balance effectiveness of the operation versus associated risks. We usually talk in terms of "Bougie Size" when referring to sleeve size. This relates to the tube-sizer that is passed into the stomach to guide the staple line formation. Most surgeons use a Bougie size of approximately 40 French for a sleeve gastrectomy. This correlates to the 80% of stomach removal.

The Sleeve is the most commonly performed bariatric operation being performed. Great outcomes and safety. Acid reflux can be an issue. The Gastric Bypass patients tend to have slightly better weight loss and metabolic improvement over time.

The issue of epilepsy medication and absorption after a Sleeve or Gastric Bypass is still being answered. I probably wouldn't choose a particular operation based on seizure medication usage. Your neurologist should be able to monitor and adjust medication, if needed, regardless of your operation choice.

Our experience has been no issues with seizure medication with our epilepsy patients despite the risks of altered bioavailability related to decreased acid, malabsorption, and decreased transit time.

It's not uncommon to have an umbilical hernia. Your Bariatric Surgeon will commonly utilize an umbilical access to perform the sleeve operation. When this portion of the operation is completed, they can repair the belly button hernia.

First Point: Revisional Bariatric Surgery is not to be taken lightly, and regardless of which operation you undergo, the risks are greater. The should be done in concert with the dietician, psychologist, and surgeon.

Second Point: You need to find out what your anatomy is. I recommend returning to your Bariatric Surgeon and obtaining an UGI to see what the shape of your stomach looks like. This will help in choosing what, if  any, operation is appropriate.

Third Point: If the operation is for failed weight loss, why would you repeat the same operation that isn't working? Discuss with your surgeon the option of Duodenal Switch, or maybe a Gastric Bypass.

Surgery is never 100%, and patients can experience weight regain. You're not alone.

Try contacting your Bariatric Center that performed the original procedure and see what their recommendations are. You will most likely need an evaluation that will look at behavioral issues as well as possible mechanical causes. An UGI Swallow X-ray will help determine appropriate options.

Most surgeons offer revisions in what they are most comfortable. Be sure to evaluate the surgical options of (1)Re-sleeve, (2)Gastric Bypass, and (3)Duodenal Switch.

Sleeve Gastrectomy can be considered for individuals with Type II Diabetes and BMI >30. Not all insurance companies follow this guideline. In general, individuals with a BMI >35 and with other weight related health condition may be considered a candidate.

For these types of cases, my OB/GYN colleague will join me during the operation to do a quick tubal ligation. Adds maybe five minutes to the operation and does not increase your risk of complications.

I think this is on the low side. The price is highly variable. Just make sure you go to a program that performs these adjustments on a regular basis. The right kind of needle in an office-based setting. I would avoid fluoroscopy (X-ray).

Most of the time, your surgeon can adjust your medications to control the nausea symptoms. In addition, checking in regularly with the Dietician can help in optimizing your diet. In rare cases, patients just can't tolerate the Gastric Balloon. You always have the option of early removal. In my experience, closer follow up with your support team can avoid early removal. Gastric Balloon is contraindicated for people who are pregnant or breastfeeding. If you are no longer breastfeeding, then you may be an appropriate candidate. See your Bariatric Team to make sure you qualify.

If you are looking for a friendly, professional surgeon, with a great staff, you should visit Aloha Surgery. I was fortunate that I found Dr. Fowler via the vast internet. I trusted my gut instinct and made an appointment. Upon meeting Dr. Fowler, I knew he was the one who was going to repair my hernia. He described the various methods in repairing hernias, the amount of post surgery expectations, and the probable minimal scarring. Everything sounded comforting so I requested he move forward and lo and behold a week later, I was at Castle Medical. The finest nurses and people at Castle made me feel very special and well cared for. Another week later, I had my follow up meeting at Aloha Surgery in town, and with no waiting past my scheduled appointment, saw Dr. Fowler for the last time.
