It would be best for you to see a board certified dermatologist or plastic surgeon in person for an evaluation of how your face moves, check the skin and structures of the face under different lighting, and to talk about your goals. Fillers are usually a good option to start with, along with some preventative neurotoxins such as Botox, Xeomin, or Jeuveau. I would definitely recommend seeing an oculoplastic surgeon for treatment of this area. A lot can go wrong with filler placed directly under the eyes, and only a very skilled expert should do any further work on that area. You will need to have whatever filler is remaining dissolved with hyaluronidase if you had a HA filler placed originally. Make sure you know what product was injected so your surgeon can come up with a management plan. Fillers that are not hyaluronic acid, such as Radiesse, Sculptra, and Bellafill, cannot be dissolved. Thanks for your question. Definitely multiple types of fillers can be injected in one appointment, but your dermatologist might want to do one area at a time to see how the first treatment settles. Sometimes you might not need as much as originally planned if you get a lot of improvement from one treatment. Thanks for your question. I would definitely ask your dermatologist about options. I would think that stopping the birth control and seeing if the spironolactone is keeping the acne at bay enough for you would be a very good option. Also having a topical regimen will be helpful. Thanks for your question. I am not sure where you read those long term effects of fillers - that is simply not true. Maybe the author meant that use of neuromodulators, such as Botox and Xeomin, can cause some muscle atrophy, but I have not heard of fillers doing that. Anyway, I would recommend a consult with a board certified plastic surgeon or dermatologist with experience in revolumizing the face. They will be able to assess your face at rest and in motion and help come up with a treatment plan for you with your safety and best outcome in mind. The good thing about the hyaluronic acid fillers is that they can be dissolved with hyaluronidase if you don't like it. Please see an expert, and best of luck!

How deep are DAO injections?

Thanks for your question. Treating the DAO is a very simple and quick procedure with great results when done properly. The needles are very tiny and the medication is injected quite superficially in the muscles of facial expression, so not deep at all. Thanks for your question. The answer depends on whether the area in question is tethered to the underlying tissue or just skin that is dimpled in . You could always try Botox to see if it helps and the good thing is, it wears off after a few months, so you are not making a long term commitment if you don't think it helped. The forehead can take anywhere from 8 units to soften, up to 20 in some pateints with very strong frontalis muscles and a high forehead. Ask your dermatologist about microneedling and laser procedures as well. Please see an expert, and best of luck.

Thanks for your interesting question. Any provider can dilute the toxins to a concentration they are comfortable using. Some studies in the dermatology literature are showing that higher doses of total toxin have a longer lasting duration. You can also try Xeomin or Jeuveau which are slightly different but essentially have the same efficacy and duration as Botox. Ask your provider for options and if they are well-trained, board certified in dermatology or plastic surgery, and have a good reputation and continue to do training and research, they will probably be able to discuss the risks and benefits of different concentrations of toxin and what your choices are. Thanks for your question and helpful photos. I honestly think you look great and I would not dissolve the filler unless it caused you a lot of distress. It will eventually be metabolized, and in my opinion it does not look obviously migrated. Take care!

Thanks for your question. Accutane is effective for pretty much every type of acne. Depo Provera will not "counteract" with the Accutane, but might make your acne more stubborn to treatment. You might want to explore different contraceptive options with your dermatologist. Or, you can see a board certified OB/Gyn physician that can discuss long term contraceptive options with you. One medication to consider is spironolactone, which helps in many cases of hormonal acne. Please see an expert, and best of luck!

Praise - I absolutely recommend Dr. Paul

I am approaching the end of laser tattoo removal, and I have nothing but praise for Dr. Paul. I saw two providers prior to finally seeing Dr. Paul. I wish I had found her from the beginning; she is professional but friendly, has a wealth of knowledge, and does an amazing job. I know if I would have seen her initially, my tattoo would already be removed. She has used strong treatments, without any scarring, to limit the number of sessions I will have with her. Prior to seeing Dr. Paul, I had over 10 treatments on a small black tattoo that Dr. Paul would have been able to remove much quicker. I absolutely recommend Dr. Paul for tattoo removal, and I am sure she does all types of procedures beautifully.
