Look at their before and afters. IMO rhinoplasty surgery is a sub-specialty onto itself. Neither plastic surgeons nor ENT's have a monopoly on good results. Ideally you want to find someone who is a rhinoplasty specialist and who does mostly rhinoplasty surgeries. That person could be a board-certified facial plastic surgeon or plastic surgeon.

In many cases, additional procedures can be performed at the same time as a cosmetic rhinoplasty to improve nasal function. I do this routinely for our patients and includes: fixing the septum, placing spreader grafts, preserving the lateral alar cartilages, lateralizing the inferior turbinates, and a number of other effective procedures. IF these "breathing improvement procedures" are not done, then a large percent of patients will likely have a decrease in their nasal function due to the reductive nature of most rhinoplasties. Also, making the nose too small can created nasal obstruction. 2-weeks is way too early to analyze and judge rhinoplasty results. This is a rough schedule of milestones for rhinoplasty analysis. 2-4 months: starting to get an idea of the early result, 6-9 months: skin shrinkage and decreasing swelling giving more hints of the final result, 12-24 months: nose has sufficiently settled in most cases where the nose can be evaluated, and if needed revisions can be performed, 2-4 years: still some subtle contraction of the skin and more subtle changes to the nose. Rim grafting can feel quite natural when typical thin septal or ear cartilage grafts are used. The use of rib grafts or thicker septal cartilages in the alar rim can result in sometimes palpable or stiff alar rim. It also depends on the size and orientation of your alar cartilages, skin thickness, and other factors. But, in general, most alar rim grafts feel quite natural. Based on the pics you are probably good candidate for buccal fat removal. However the real test would be for a surgeon to examine you and feel the cheeks and your bone structure to figure out how much of the cheek fullness may be due to the deep buccal fat vs other factors. Being as close to your ideal or normal weight will help your surgeon in removing just right amount of buccal fat, and not too much or too little. Having said that, buccal fat volume doesn't really change much with weight loss, so you could get it done before reaching your ultimate weight loss goals. Technically you could have the buccal fat removal a month after rhinoplasty. Having said that, I would recommend waiting 2-3 months so that any rhinoplasty related facial swelling settles down and your surgeon can use better judgement on how much buccal fat to remove. You are good rhinoplasty candidate. I would clearly communicate with your surgeon your aesthetic goals. Your nose is also crooked and will need to straightened. One important thing to keep in mind...overly narrowed noses may result in permanent and serious nasal breathing problems. As such, you need to figure out with your surgeon a nice in-between size reduction, other functional procedures (like septoplasty) so that you get the prettier nose you want + preserve your nasal breathing function. Looking at the pictures one has to balance your desire for more tip refinement, with the real possibility of nasal obstruction and compromise. You see, to a certain extent when you narrow the tip, you are also narrowing the inside of the nose which could cause serious breathing problems. So think about that carefully and request a conservative degree of change from your surgeon to lessen that risk. Also, you may need spreader grafts to prevent nasal obstruction.
In general, narrowing the nose entails: removing some cartilage, and/or using stitches to narrow the tip, and moving the bones closer together. This can be done either using the closed or open rhinoplasty approaches. All the best, Dr. John Vartanian from LA.

I have been coming to Dr Vartanian for about three years for Botox and fillers. During my most recent visit, I expressed concern over my jawline and inquired about any solutions that he’d suggest.
