Thank you for your' question. A thread lift does not prevent you from having a facelift even if permanent threads are used . Permanent threads tend to cause some minor issues over time and the effect of thread lifting is fairly short term (especially if absorbable threads are used) but provided they are expertly placed by someone who fully understands the anatomy of where they are putting them they are usually not too much of a problem for the surgeon performing a subsequent facelift. I have frequently encountered threads in deeper tissues than the original doctor would have intended when I have performed a subsequent facelift. On a few occasions the thread and small sleeve of surrounding scar was adherent to branches of the facial nerve and great care was required to dissect it free and avoid a facial palsy. I feel sure the doctor who inserted the thread would not have intended to place it so deep but it goes to show that whatever technique is employed for facial rejuvenation the experience and judgement of the operator is paramount.

You have had excessive and unbalanced removal of central submandibular fat especially posteriorly without satisfactory tapering of this laterally combined with inadequate redraping of the skin and platysma laterally. Was your operation just done from the incision beneath the chin? A combined periauricular and submental approach would be required to fix this.

Deep plane facelift and Invisalign?

The Invisalign treatment will not interfere with the facelift, It is fine to proceed with both. You should avoid taking the Invisalign scanning photos using the cheek retractors for 2 weeks post op but you can keep your existing retainers in during that time.

The problem you describe is a well known complication of a poorly executed lower blepharoplasty particularly if performed on someone with looseness of the lower eyelid ligaments or protruding eyes. Without seeing any pictures it is difficult to determine what could be done but there are potential remedies to recruit more skin into the lower eyelid or support the lid position by adjusting the ligaments.

Hi and thanks for the question. Whilst it is difficult to comment on the basis of just two photos it does appear that you have a combination of upper lid ptosis (the lids are drooping over your pupils ) and excess crept skin particularly over the outer third of your upper eyelids with hooding in this area covering the outer corner of your eye. This could be improved upon with further surgery subject to a proper evaluation with a plastic or oculoplastic surgeon who has expertise in this area. It would help if you could provide the operation note from your previous operation (you are legally entitled to this) as the previous surgery technique may have some bearing on what can be done in future especially if the first operation included some form of ptosis correction. Photos from before the previous surgery would also be helpful. Finally it is important that any Botox has worn off before making a final evaluation of upper eyelid position as weakness or imbalance of the forehead muscles can have a major influence on the upper eyelid through changing the eyebrow position.

Hi and thank you for your question. In my opinion you would definitely require both approaches performed at the same operation. Your platysma bands are tight and would need to be released and repositioned through the incision under your chin and the loose skin of your neck redraped laterally and trimmed at the same time. If the operation was just  limited to the incision under the chin it could make things worse as tightening the platysma muscle by pulling it to the centre tends to pull more neck skin inwards resulting in more looseness. By combining the midline platysma repair with outward and upward repositioning and trimming of the skin through incisions around your ears a much more comprehensive and harmonious result can be achieved.

thanks for your question. You have low set brows with a horizontal orientation, a high very smooth forehead, high medial lid folds and just a small amount of lateral upper lid hooding. You mentioned that you get Botox to your frown lines which has eliminated these nicely but sometimes botox can lower the brows if it is injected into the frontalis muscle of the forehead. A brow lift may help but the evaluation of your brow position should only be done when the Botox has worn off completely which is about 4 months post injection. There are several different ways of performing a brow lift ranging from incisions behind your' hairline through to just above the brow or even through the eyelid and the surgeon should be able to discuss all these approaches with you. I would advise you to seek out a consultation with a plastic  or oculoplastic surgeon specialising the the face and arrange an appointment timed for when the botox has worn off.

Thanks for your question. The photo you have provided with your head turned and flexed is creating an exaggeration of the folding under your chin and across the jawline so it is difficult to provide a definitive recommendation based on this. You appear to have excellent skin and full subcutaneous tissues so I doubt that fat injection would be helpful. Most probably lifting your lower cheek and jowl area plus the region just below the jawline would reveal the jawline and rejuvenate the contour of your neck. There are several techniques for doing this and determining the most appropriate one would require a direct face to face consultation. You should seek out a plastic surgeon who specialises in the facial area for a proper evaluation.

From the two photos supplied it appears that you have asymmetry of the nostrils including the nostril floor due to deviation of the lower part of your nasal sepal cartilage. The pair of wing shaped nasal tip cartilages sit over the lower end of the septum a bit like a hat sitting on a head . When the septum is deviated the overlying tip cartilages can twist and deflect into the nostril causing asymmetry. Straightening your septum and repositioning the overlying tip cartilages should correct this. Your photos only show a limited view and a full examination would be required to determine if there are any other issues

Fat transfer is commonly performed as part of facial rejuvenation. It is used to enhance the volume of tissue in areas that have thinned out with age. There are some risks including the possibility of embolisation if the fat is accidentally injected into an artery. This can lead to the fat passing along the artery causing blockage and therefore damage to whatever the artery supplies. Probably the most feared complication would be embolisation of the retinal artery causing blindness in the affected eye. Whilst this sounds alarming the risk is very low especially if the injection is performed carefully. The question of whether you really need any fat injection is a different one and would require photos or a direct consultation. Ultimately the choice is yours, your surgeon can make recommendations but you should only proceed if you are comfortable with the advice given.

Couldn't be happier with the results - I feel so rejuvenated

It has taken me a while to finally write my review as I wanted to wait a year to see the final results as I do believe the face needs time to settle completely. At the beginning of my journey embarking on finding the best surgeon, it all seemed quite daunting. From one surgeon to another (one which was recommended) and another after that, I finally met with Dr Hansen and knew he was the surgeon to perform such an important procedure for myself. Having already had the comparison's from the previous surgeon's, the questions that I asked and the answers and recommendations Dr Hansen provided confirmed in my mind having done my extensive research, he was the surgeon for me. As far as I know Dr Hansen is the only surgeon in Perth that solely operates on the face. Cutting a long story short, one year and four months post op, I couldn't be happier with the results as a 55 year old woman I feel so rejuvenated. I have some close friends that know I underwent this procedure and are considering the same and if they do decide they will definitely be going to Dr Hansen.

After much research I decided to see Dr Matthew Hansen, to discuss how I could look and feel less tired around my eyes. Dr Hansen listened to my concerns and suggested an upper blepharoplasty would achieve my goal. He was extremely professional, particular and explained in detail what to expect. He also made me feel like he understood it was an important decision for me. He didn’t upsell at all. I decided to also go ahead with the lower blepharoplasty. I am thrilled with the outcome. I am so thrilled at the results of my surgery. I knew I was in good hands with Dr Hansen as know other surgeons refer their family to see him for cosmetic facial surgery. After 4 consultations and 10 years of procrastinating, I finally made the decision to move forward. You would have to look very closely to see that any surgery had been performed

From my first phone call to book a consultation for BCC removal and repair, to the follow-up care post-surgery, every experience with Matt Hansen and his lovely staff (Laurie and Patsy) was beyond excellent. I felt supported, listened to, informed and cared for throughout. The cancerous growth was wide and deep, located on the side of my nose, near my eye. I am two weeks post-surgery (with clear pathology) and I kid you not…you would have to look very closely to see that any surgery had been performed, he’s THAT good. “Best hands in the business” was what my referring dermatologist said…So true. We are very lucky to have Matt here with us in Perth.

Excellent care and results so far! 10 days post-op and still somewhat swollen

I am only 10 days post surgery and still somewhat swollen, but already I can see the results and look forward to them being even better once the swelling has gone. Dr Hansen gave me excellent care before the surgery, did not rush me and answered all my questions (plus prompting me with a few I hadn't thought of!). While I was in hospital he was calm and reassuring before the operation, and then checked on me several times while I was still there. The operation went well, I was surprised how quickly I bounced back, no pain just a bit of discomfort - I didn't even use the painkillers that were prescribed as I didn't feel that I needed them. I have no hesitation in recommending Dr Hansen - and his nurse Patsy - for their excellent skills and care. I am excited to see the final results and will post before and after photos in a few weeks when I am at my 'new normal'. Thank you so much!
