Congrats on the ongoing weight loss. Keep going. As a result of this level of massive weight loss, you will be healthier, feel much stronger, and have much more energy. The trade off is lots of extra skin...everywhere. The lower Body lift (Belt lipectomy) is likely to be your first procedure which entails the circumferential mid sectional skin removal, elevation of the lateral (outside) thighs, and buttock lift, all in the same procedure. The breasts, inner thighs, arms and/or neck will be addressed later, as per your priority choices.

I cannot tell if you are a good candidate or not because I don't know your medical circumstances, your BMI (ideally 29 or less), your smoking status, your expectations, or your ability to take a couple weeks off. Addressing only the image submitted, the procedure that address the abdominal contour and scar would be an abdominoplasty +/- liposuction of the abdomen and flanks. The midline scar, however, is quite high and some of the top portion of the scar would likely remain.

Every procedure causes scarring, even the two previous liposuction cases. The abdominal skin will be stuck down to the muscle fascia and, on occasions, will not re-drape as smoothly as skin that has not been previously suctioned. The muscle tightening will enhance your mid sectional contour significantly. Take the plunge. As thin as you are, you will never get the hourglass figure without the procedure.

I don't like to use or to offer the term "mini" tummy tuck. Patients want a minimal scar and minimal price but also want maximal results that are the result of a full abdominoplasty. The two processes are incompatible. In the photos above, you will need muscle plication that extends nearly all the way to your xiphoid for optimal contour, even though you may still have stretch marks. Liposuction to the flanks would also be a very nice adjunct.

The best way to remove excess skin is to remove the excess skin, as in tummy tucks. Short cuts have short lives results and usually compromise shape.

In contradistinction to Caucasians, as African Americans, we often have lower dorsums, wider noses and thicker skin. So the usual remedy is to build up the dorsum, narrow the nose and suffer with the thick skin. Because our skin is thicker, we, like Asians and some thick skinned Hispanics tolerate on-lays (out of the box) which extrude in most Caucasians and other thin skinned people. I have never had a prosthetic on-lay extrude from a Black patient but I have removed many from Caucasians, Latinos, and thin nasal skinned Asians. If you want autologous augmentation (your own tissue) the fascia (tissue underneath the scalp), wrapped around ear cartilage, in my opinion, is a better option because it doesn't hurt, doesn't warp, cannot cause pneumothorax (lung collapse) and a doesn't put a permanent scar of the chest, which may be important more so in females. Additionally, if you pull the dorsum forward enough, you may find that the bottom of the nose narrows enough to avoid an incision used to otherwise narrow the nostrils.

In general, silicone implants are superior to saline implants. However, there are three scenarios in which saline implants are preferable. The first scenario is in the patient who has a narrow budget and nice soft tissue coverage. Saline costs less than silicone implants. We, as plastic surgeons, achieved great results when silicone implants were routinely off the market in the 90s. The second scenario is in patients such as yourself in which you'd want to "see" before committing to what you might look like with no implants. You may love it. You may hate it. But at least you would be able to make an informed, visual decision before opting to a larger procedure. and your decision is reversable. The third is the patient who has gained weight over many years and the breast are now too large. She doesn't know if she wants smaller implants or, if they are still too large when the implants are deflated, she may choose to have NO implants at all. Best advice is to deflate them in the office using local anesthesia. It will be obvious in a very few weeks if you love the new look (and simply want them removed) or whether you still want "a little somethin" in which case you may opt for a smaller size.

So sad to about your dissatisfaction, Kylie. Breast implant removal is a very easy and, in the right scenarios, rewarding operation. Simple removal is performed in the office, using local anesthesia only. Occasionally the patient may ask for an oral sedative but unless she is s nervous wreck , it really isn't needed. There is no need to miss meals. You do not need a driver and it can be performed without taking days off of work or your usual activities. The existing incision suffices without adding new incisions. No drains are needed. And because it is performed without an anesthesiologist, the cost are significantly reduced compared to the cost of you previous primary augmentation.

If you ask 10 surgeons, you may get 10 different answers. But in the end, YOU have to be comfortable with the surgeon, his/her rationale for the approach given, the risks, the timeline and the expectations for the chosen sequence. My approach would be to do the lower body lift which includes the tummy tuck, and lifts the lateral thighs and buttocks in your first operation. This is the biggest operation and the most bang for your buck. The inner thighs (medial thigh lift) , which are also liposuctioned, are reduced last. You did the hard part by loosing such massive weight. Now choose the flight that meets your expectations ( and priority) and leave the rest to the surgical team.

Tummy Tuck or Body Lift? (Photo)

Asked By:AnonymousANSWERS (1)
The distinction between the two procedures depends on the condition of your lateral thighs and buttocks, both of which are covered in the photos submitted. With that much weight loss, it is highly likely that a simple tummy tuck and lipo will be insufficient because the left over skin laxity is circumferential both vertically and horizontally which makes the body lift more reasonable... even without seeing representative photos. However, I have seen several patients whose thighs and buttocks look great after massive weight loss and those patients do well with smaller procedures.

True to Their Word

Dr. Larry Leverit and his team are exemplary. I have been a patient for procedures and skin care both since 2013. They are masterful at their God given crafts from the beginning to the end, and then some. I have friends and family that have been going to Candice, their skin care professional, for years. She is the reason they look as refreshed, renewed and as young as they did from their first encounter. Candice is not in it for the money. She is in it for results. She will give her honest opinion as to whether a treatment would fulfill expectations, and if not, why. At which point she provides alternatives. The Leverit motto is, “ The Beauty You deserve”. They are true to their word and do not fall short of their commitments.

Long overdue review - thrilled with the results of my surgery

I really cannot say enough great things about this whole process. It's been over a year since my first ever surgery, which was a breast augmentation. This review is long over due! Okay, from the start: the office was professional from the initial appointment all the way through to the end. I made my appointment and loved the text reminders. My consultation was great. The staffs and Dr. Leverett asked great questions about what I wanted and many health questions which made me feel like they were concerned not just about what I would look like but my overall health. Dr. Leverett came in and was very professional. He answered all my questions and was not in a rush and really wanted to understand what I wanted. After the consultation (which was no charge) Amazing! They were very clear about the cost and breakdown of that cost. The pre-op process was great and they made sure that I had absolutely all the information I needed to be fully prepared before my surgery date. They answered all of my questions, made sure I had my medications and gave perfect detailed instructions about what to do and what to expect. The day of surgery was completely smooth and check in was easy. The staff was so nice and they made sure I was comfortable. They took EVERY precaution from compression socks, to Tylenol and took care of every minor detail. Dr. Leverett saw me personally before surgery to do a final confirmation of what we were doing and made sure I had no more questions. I was nervous but felt safe in their care. After surgery they were so connected and checking in on me, making sure I was doing well. Every one of the follow up appointments were so great- so easy to book, and always very thorough information and just great care from every single staff member. I am thrilled with the results of my surgery. Down from the looks and healing. Both surpassed my expectations. I am absolutely overjoyed with every single aspect of this incredible team. Thank you so so much!

So happy with my results and my decision to go with him

Dr. Leverett is so warm and welcoming. I had several consultations to have my 12 year old saline implants exchanged for silicone. The entire process was seamless and easy. Everyone in his office was available to answer any questions I had and were the greatest hype woman a girl could ask for. I am so happy with my results and my decision to go with him.

Thrilled with my results - only regret is not doing it sooner

Couldn't have dreamed of a better experience. From the consultation to post op, the surgeon and staff were amazing! Dr. Leverett was very thorough and patient and went over in detail all of my options and the office staff made it easy to schedule my appts and answer all of my (many) questions. Surgery and recovery was a lot easier than I thought it would be and I am THRILLED with my results. My only regret is not doing it sooner.

Elated! Breast lift after 220 lb weight loss & did not want implants

I met Dr. Leverett and his staff in May 2022. From the beginning everyone was very open arms, caring and willing to help. I had so many questions and had 3 previous consulations under my belt. First Keri spoke with me during my consultation to better understand exactly what I wanted. She then took pics and I had my consult, 2.5 hours, with Dr. Leverett. He first congratulated me on my 220 lb weight loss and then listened very intentionally to what I want to accomplish with my body. I did not want implants. He examined my breast and showed me what he would be able to accomplish with my own breast. He agreed as well I had enough great tissue and did not need implants. After our consult, I spoke with the financial advisor and began preparing to pay for my surgery. My surgery was done on June 28, 2022 and I am elated. I highly recommend Dr. Leverett to anyone looking for this procedure or any others. He has been a Dr. in plastic surgery for 25 years and still cares and has a wonderful bedside manner.

They are beautiful and very full now - from an A to a DD

I received a breast augmentation from Dr Leverett back in May. I have always hated my breasts. I had several consults with surgeons in the area and SO SO SO happy I chose dr Leverett!! My self confidence has shot way up. After having kids, my breasts were deflated and sad looking. They are beautiful and very full now. I went from an A to a DD! The recovery was a breeze for me. I had kind of a difficult case due to some pre existing health issues and dr Leverett did everything he could to ensure that we did this safely and effectively. I knew I was in good hands. I honestly can’t say enough good things about dr Leverett and his team. Kerri was also a pleasure to work with and she was so accommodating and helpful. This was truly life changing! I’m looking at more procedures with him in the future , when time permits!

$13,000Tummy Tuck

So pleased with my results - why did I wait so long?!

I've wanted a tummy tuck for many years. I met with a surgeon in Bangkok a few years ago but didn't trust how dismissive he was about aftercare needs. I found my surgeon here. I was told that I was a good candidate for a tummy tuck because my weight had been stable for a while, I was in the correct BMI range, and my issue was simply loose skin from weight loss (no need for lipo). I went to my primary care doctor to get some pre-medical checks done. Once those were all checked out (everything must be done in 30 days from those test results), I scheduled my surgery. I went to the pharmacy about two weeks before to get the medications I would need. I also bought some compression garments. The surgery itself felt like a breeze. I woke up and felt great (love those meds!). For me, the most annoying part of the recovery was the drains. The drains just felt scary to me. When I was able to take my first shower, I was just really scared that I was going to do something wrong. I was SO happy that I only needed the drains for a week. Getting those out felt like a huge mission accomplished! I wish that my surgeon had been with me when I did the big reveal the first time. Taking off my compression garment after three days to take my first shower was kind of scary. I have to be honest that when you see your scar the first time you realize what major surgery this is. It's no joke! I slept a ton the first couple of days. I focused on hydration and was not super hungry. By Day 3, I no longer needed my pain meds. I was sleeping normally. I even went out that weekend for a little while. It sucked that I couldn't drink alcohol for a week after but it was OK. Of course, you aren't standing up totally straight after but it still felt wonderful to get out! Beyond that, my recovery has been a breeze. I am now about two months after. I have been cleared to exercise again although I haven't started (need to get back into the groove). I am walking almost straight again. My stomach still feels tight but it is getting better (alternate with hot and cold water in the shower). I am still working on my scar. I bought the Embrace bandages. Seems too early to know if they are really doing much. I hope so as they are expensive. When you budget for a tummy tuck, I would recommend adding in an extra $2k. There are so many hidden fees that you don't see. One thing that I need to work on now is a new wardrobe for my new body. From the creams for the scar to everything else, costs do add up. I am so pleased with my results. I cannot believe how smoothly my recovery has been!

Nipple Reduction

Great doc, great practice - felt informed, comfortable and confident

My experience - encompassing a fairly straightforward nipple reduction - was first rate in terms of service, comfort, and outcome. From initial consultation to final check in, I felt informed, comfortable, confident, and in control!
