I really suggest an exam to define the size of the implant. But in my personal opinion this type of chest wall requires a implant with a good size and projection to make the diference.

To reduce the nose wide a combination of procedures should be done:
1. alar plasty
2. osteotomies
3. Tip projection

Is very good to discuus with the doctor all the possibilities

First of all, is very important to see your previous pictures. Is important to cheek if you have some degree of asymmetry. 

In your case a surgical procedure will be a good option. Almost all the problems con be correct in a single procedure. 

The picture is not so good. The surgical area have some edema ( natural in this fase) , equimoses in the skin. Is difficult to define if this is an hematoma or seroma. Is better to return to your doctor and cheek this.
