Dr. Vasquez is amazing. He is extremely knowledgeable and did an amazing job with my fillers and Botox (looks very natural and I'm so happy!). His staff is great too. Very friendly, helpful and kind. Dr. Vazquez is a gem. I don’t think my experience could have been any better. For starters, he seemed to be a natural at immediately ridding me of my anxiety about the procedure because he is a great listener. He aimed to fully understand me. Even when we had different opinions about post op breast size, he communicated his concerns in a respectful way, letting me know he does have my b(r)e(a)st interest in mind. Dec. 2016 I made the decision to have breast augmentation surgery with Dr. Vasquez. From Day 1, I knew I was in good hands. Vasquez and his staff were wonderful start to finish. 4 weeks later I feel great and couldn't be happier with the results!

I called the office on my 17year wedding anniversary, to set up a appointment for a consultation. I brought my husband with me to the appointment. After my consultation, my husband said I like Dr. Vasquez I believe he will take good care of you. After pushing my date back a couple of times, I set a date. This was my first time getting fillers of any sort and I could not be any more pleased with the results. I did not experience any bruising and very, very little swelling. In fact, the injection site on my left is the only area that swelled and that was because it was a little tougher to get the needle in.
I had been considering a breast reduction for a number of years but hadn't had the courage to follow through with the procedure until recently. I was concerned about down-time from work, scarring, recovery, the procedure itself, cost and whether my body image goal could be attained through weight loss as opposed to surgery. At 5'4", 130 lbs, size 6 with DD breasts, I realized that my breasts were not going to go anywhere no matter how healthy I ate or how much I exercised. In May 2016, I finally mustered up the courage to at least go to a consultation. Dr. Vasquez and his office manager Lisa are the consummate professionals. I felt so comfortable and at ease in the office that I decided it was time to take the plunge. I had the surgery approximately a month after my initial consultation and I could not be any more pleased with the result. Aside from a minor wound issue, everything went perfectly. I am now a 36B and I sleep better than I ever did before. Prior to my surgery, I liked running -- I now love it as it is 10x more comfortable without the weight pulling at my chest. My only disappointment is that I didn't have the surgery sooner.
He was my first and only experience with plastic surgery, and I am SO happy that I chose him. This was 8years ago, and I can confidently say that my breast augmentation was the best decision I ever made for my own self esteem. Besides that, I've had ZERO complications, they look even BETTER and more natural as I age (and have had three babies since the procedure, and they look WAY better than most post-partum Mommies!), and I even fell once randomly on ice 5years after the surgery and he saw me no cost, no problem, just to be sure all was well.
Based on your photos, and just limiting the answer between those two sizes, I would say that you are likely to be happy with either size, really just being a difference of only about 10%. But a jump from 400 to one of these sizes is about a 50% increase in size, which may be too large for your own anatomic dimensions, as well as the significant increase in weight, which will cause your breasts to sag or droop. I would keep these factors in mind more than the trying to decide between the two sizes you inquired about.
A full in-person evaluation and photos can help me determine if I would recommend liposuction as a primary means to reduce the bulk between your thighs, and your doctor may have been correct. That being said, plastic surgery is both an art and a science, and this doctor may not feel he can achieve your expectations. You may find others who will offer you this procedure based on their experience, so it may be worthwhile to get a few opinions. But I would caution you to not forget your first consultation and understand why he/she refused to perform this procedure on you.
Based on what I see (only a live consultation can really determine completely), while you have a snoopy dog deformity, it appears that you have some capsular contracture in addition to inadequate treatment of breast ptosis (drooping), which really necessitates a lift in addition to augmentation. Minor ptosis, and snoopy dog deformities, can be improved with a dual plan technique, but significant ptosis can not be corrected with a breast augmentation alone.
I would want to know what your fears are about "breaking" the nose, which we call lateral osteotomies. I do think that it is possible with the fairly minimal dorsal hump that you have, but the top of the nose would end up looking more square. I think your concerns are very common, but I think it is better to frame the question as, what do you really want as your final outcome? Plastic surgery is generally not the place to compromise, and there has to be a certain amount of trust in the process and the relationship in order to get the "best" results.
The great thing about plastic surgery is it becomes a balance between art and communcation, so you get get different answers from different surgeons. You will have to trust your own gut when it comes to the surgery that is right for you. Looking at all the answers, one option to consider is to have an augmentation without lift and expect that you may need to do a lift later if you feel that the implants were not enough to give you the lift and projection you desire. We all understand that a lift creates more incisions and scars in order to get the shape that is needed, however you are the one that has to walk in your shoes and be happy with your choices.
