What are my options for caudal septum deviation?

It is possible to straighten your caudal septum deviation to improve the symmetry of your nostrils. It is also possible to address this alone and leave the rest of your nose as is if you prefer. Without examining inside your nose it is hard to say what method of treatment is best for your situation. If the caudal septum deviation only includes the very outer edge then one could consider trimming that small portion, but in my experience these types of deviations extend a bit further back to include more than just the leading edge. I much prefer to maintain the septum length and don't recommend resecting the caudal septum in most situations. Larger caudal septum resections can cause the columella to retract upward (which would change the appearance of your tip). Although it is more complicated a better approach involves repositioning or reconstructing the caudal septum so that it is located more in the midline. A great way to start the process of finding the best rhinoplasty for you would be to review various surgeons' before and after photos to get a better idea of their aesthetic. This should help you get an idea whether they tend to only go for drastic changes (this may be their patients' goal) or if they can also do more of a finesse rhinoplasty. Additionally, I find that the digital simulations that I do with my patients during the consultation process is very helpful in making sure that we're on the same page with regard to what changes (and to what degree) are desired.

Is Septorhinoplasty safe if you have a heart condition?

You'd want to get clearance to have septorhinoplasty by your cardiologist as that's their area of expertise. It is possible to have surgery in a hospital setting if that's what is recommended. I have privileges to operating in both Seattle area surgery centers and hospitals, for example.

What happens to the collapsed upper lateral cartilage when spreader grafts are placed?

Since the spreader grafting is placed between the upper lateral cartilage and septum the upper lateral cartilages will be moved further away from the midline (laterally) after spreader grafting. Sutures are used to secure the newly positioned upper lateral cartilages to the spreader graft and septum so it won't move out of place. Can you do a revision rhino without op reports from 1st? Is it possible to have a positive revision outcome in this situation?

Having prior operative notes are very helpful before doing revision rhinoplasty, but they aren't required. It can be difficult to procure these outside records, especially if they're from many years ago. You'll want to ensure that your revision rhinoplasty surgeon has extensive experience and can adapt the plan depending on what is found during surgery.

Would my nose be a difficult case for rhinoplasty? (Photos)

Yes, I can see how your profile could be refined with rhinoplasty surgery. This could create a less full bridge on side view. Besides your upper bridge bump you also have an area of fullness in your supra tip region (the area just above your tip) that could also be addressed at the same time. What is a functional rhinoplasty?

The goal of functional rhinoplasty is to improve nasal breathing/airflow. I prefer using this term to imply that nasal valve repair is being performed. This can also potentially involve more complicated septum reconstruction. A simple septoplasty isn't normally considered a functional rhinoplasty. The functional rhinoplasty term also indicates that there isn't a cosmetic component, though it is common to perform both functional rhinoplasty and cosmetic rhinoplasty concurrently in patients depending on the goals. It's hard to know if your ENT is qualified to do functional rhinoplasty. A lot depends on their experience level. You can check out my web link to learn more about functional nose surgery in general.

Does ENT perform rhinoplasty or should I ask for a referral?

Most general Ear Nose Throat doctors don't perform rhinoplasty. It is very common to have both cosmetic rhinoplasty done at the same time as nasal breathing improvement surgery like you're looking to do. You could find a facial plastic surgeon who is board certified in both ENT and facial plastic surgery who would be able to treat both issues.

Is there a limit to how much you can narrow the bridge in an osteotomy?

Yes, there are limitations as to how much bone narrowing can be done with osteotomies. I find that performing medial, lateral and intermediate osteotomies can help in maximizing how much narrowing is possible when someone has extreme baseline bone width. Does endoscopic septoplasty involve less side effects than non-endoscopic septoplasty?

Endoscopic septoplasty is not a common or standard approach to perform septum surgery. In my experience using an endoscope for septum surgery is not normally needed and doesn't reduce the side effects to the procedure. The endoscope is a small telescope that gives the surgeon a magnified view inside the nose and is typically used with sinus surgery, but since the septum can be worked on with just a headlight and speculum we don't normally need an endoscope. In either case an incision would need to be made to do the septum surgery and the endoscope wouldn't be expected to create less side effects. Septum surgery is a very common procedure for many otolaryngologists and in my experience has low risks and side effects. You can learn more about septum surgery at my web reference link below.

After 3 surgeries... It’s the nose I’ve wanted all my life!

I had my first rhinoplasty done 12 years ago. Long story short, my nose looked beautiful but I botched it myself 3 weeks after I got it done.Two years later I had it done by the same surgeon and he made my nose look so bizarre. Out of panic, I had it done again 7 months after that by another surgeon. They made it super narrow, short and compromised my breathing. I felt that my nose was way too small for my face. As far as revisions go, these doctors did not know what they were doing. I decided to let my poor nose be for a while. Fast forward almost 10 years later and after many consultations, I felt that Dr. Lamperti was the person to make it right. From the get-go, I knew he was the one. He listened to my whole story, listened so patiently to what I wanted my nose to look like, answered every single one of my questions, and gave me a realistic idea of how he could help me. I flew from La to Seattle after our virtual consultation. Nothing but patience and listening from Dr. Lamperti - it was honestly so refreshing after multiple consults with LA and NY doctors who seemed full of themselves and who didn’t really let me do any of the talking. I had my surgery with doctor Lamperti on March 12, and it was the best decision EVER. He took rib cartilage which is said to be much stronger than ear or donor cartilage and used to give my nose more symmetry, projection, volume, and to prevent my Nose from collapsing when I inhale.The results are honestly better than I could’ve imagined. It’s only been a week since surgery, and swelling is sure to go down, but I already feel so much confident and happier with how my face looks... I’m even starting to shed tears of joy as I write this. I will continue to share updates on my healing process!

Liking my Results So Far!

I am writing this 6 days after my Lip Lift procedure and so far I am very happy with my results. Dr. Lamperti and his staff were very attentive and quick to respond to my questions. I wanted to give a little information about how things went. I started with a 21 mm philtrum and we decided on taking 6 mm off. I was conservative because I did not want to end up with an unnatural look. The procedure itself took 1.5 hours including time to numb you. The only difficult part is the needle for the nerve-blocking shot. Three very large sticks and a burning sensation, but after that, I was practically daydreaming. Day 1: Cartoonish amount of swelling in my lips, nose and face. After the nerve blocker wears off there is a burning sensation. I took an Oxycodone and slept. Day 2 - 3: Nose, lips, and face still swollen. Tylenol and ice suffice for pain. Day 4-5: Nose and philtrum swelling way down. My lips look beautiful! Tylenol for the discomfort (pain isn't the right word). Day 6: Aww my lips don't look as 'fresh off the factory floor' as the day before, but I still like them. They are cute and are more symmetrical with my eyes and cheeks. I'm inspired to lose weight to help them really pop. I really want my stitches out because they are irritating/itchy. Two more days until removal. Updated on 9 Sep 2020: Day 8: Stitches finally out! Still like my lips. It's easier to see the difference in photos and video than in the mirror. My therapist said I look more "defined". The only problem is, half of my stitching marks look raised, inflamed, and Frankinsteinish. I emailed Dr. Lamperti about it and he responded right away, telling me that it's normal to have a raised stitching 'line' at first and it will go down. He also said I can't wear makeup for another week, and I doubt makeup would camouflage this anyway. A warning to people who think that this procedure has only 1-week downtime. Though I suppose wearing a mask would help. I'm using Bio-Oil and sealing with Vaseline for good measure. Will update the healing procedure. Updated on 14 Sep 2020: Day 13: I started a new job today. I wasn't able to cover my scar with makeup because on one side it is too large and indented. I feel very down about it. I am hoping that the indentation is temporary as the doctor said.

Insecurity is who? I’ve never felt better about myself

Dr.Thomas Lamperti is an artist! I came into his office with a bulbous nose and a deviated septum my nostrils were originally disproportionate and I had difficulty breathing my nose was excessively long and too big for my face. He emailed me a simulation of my new nose and answered all of my questions before surgery.I am now one week post-op and I’ve never felt better about myself I’ve been given so many compliments and I finally feel like I can come out into the public without feeling insecure about my nose. This has been the best decision I have made in my life! My new nose is an exact copy of the simulation he sent me and looks very natural. Dr. Lamperti is the go-to if you care about quality and perfection.

Dr. Lamperti Exceeded my Expectations!

Dr. Lamperti is the best! I met with three other doctors before my consultation with him and I knew right away he was the right one for me. Dr. Lamperti actually listens to his patients and sets up realistic expectations while focusing on achieving an amazing functional and cosmetic outcome. He has wonderful bedside manner and explains everything in detail, he personally emailed me after my consult with more information on things we discussed, and responded promptly to any additional questions I had. I had revision rhinoplasty to correct breathing issues (collapsed nasal valve) and wanted a subtle change to refine the tip of my nose and correct a hanging columella. I’m beyond thrilled with my results and I’m only two weeks out. I’m posting a before/after photo and I will update as I continue to heal and as swelling goes down more.

Great experience! It really can take a full year to see results.

I had a total of 4 consultations with different surgeons before deciding Dr Lamperti was who I wanted to do my rhinoplasty surgery. I choose Dr Lamperti because he seemed confident, knowledgeable and experienced. I am so glad I did because I am loving my new nose. I was really nervous leading up to my surgery but the whole experience was great. Dr Lamperti and the staff were super helpful and friendly. Although I looked really scary for about 2-3 weeks, it was worth it. I finally feel comfortable getting my photo taken from my side profile! It’s been 6 months and I do still have a little bit of swelling on the tip of my nose but he did say it can really take a full year. Anyways, I would highly recommend Dr Lamperti!

Dr Lamperti has been a life-changer. I’ve always had a deviated septum, complete with breathing issues and chronic congestion. And then, in my late 40s, I broke my nose. I could scarcely breathe through my nose, and it looked significantly crooked, with a twisted tip. I consulted with Dr Lamperti because of his emphasis on both function and aesthetics, and after our initial meeting, scheduled my procedure with confidence. I am now four months post-op, and still marvel on a daily basis at how much better I can breathe (and how much less tissue I need). My nose is healing beautifully, looking better every week (I had an open procedure, so months of subtle swelling at the tip is to be expected). Dream Come True

I am So grateful for an excellent result after rhinoplasty with Dr. Lamperti. It’s quite magic to go from a witch looking nose – with the bump, projection plus a mole, to a straight, nice nose. I also had the mole removed. I could never accept my nose no matter how hard I tried but now I have to be careful not to become too proud of my nose! :) Dr. Lamperti has a very pleasant and interesting personality. The fact that he is into arts: music, photography, shows that he is a creative person and has an innate feel for aesthetic, an important aspect for a facial cosmetic surgeon. He is a great listener and understands exactly what you want and give you all the information you need. As for his surgical skills, the many incredibly successful rhinoplasties and rhinoplasty revisions speak for themselves.
