I would continue with finasteride at the recommended dose of 1mg/day as long as you are having no side effects. Any medication can cause an allergic reaction such as swelling, trouble breathing, anaphylaxis and death, but this is usually an immediate type of reaction to a new medication. You have already been on the medication for several years with no problems or side effects, so you should continue taking the medication.

At 4 months your hair should be just begging to grow. You really need to wait at least a year before you can assess your full and final results. Also, depending on your type of hair, and how short you cut it, you may always be able to see through to the scalp. I see this in patients with straight hair who shave with a number 1 or 2 guard.

I would start with medical reasons of why you are loosing your hair. Labs such as your iron level and thyroid function tests to name a few. Minoxidil 5 percent solution is your best bet, along with Finasteride if you can accept the rare risks associated with it. I am also finding PRP to be very helpful in stimulating hair growth.  A Dermatologist may be helpful to make sure some other medical issue is not going on.

I would not recommend placing your head under water. I recommend to my patients a calming spray from HairMedica immediately after surgery to help resolve the clots. Being 4 days out, Sterile saline sprayed onto the scalp every few hours would be helpful.

Hard to say what may be going on with you from a medical standpoint and the cause for your original hair loss. Important to rule out any medical issues, especially in women things like iron levels and thyroid function would be important. In men, if there are no medical issues, a hair transplant is considered permanent. Any further thinning would be from continued hair loss of the existing native hair.  That is why it would be important to continue with medical treatments such as minoxidil.
