Thanks for the question. Usually you should have some improvement a month out from septoplasty. However, there could be other factors as well such as allergies, inflammation etc. I would go back to your surgeon so they can look with a camera to see if there is any residual anatomic obstruction or if you need to put on some nasal sprays to help with the obstruction. There are multiple reasons why people can have persistent breathing problems after a septoplasty. Those causes could be from medical reasons (could have history of allergic rhinitis or medication use - too much Afrin), to structural problems such as a persistent deviated septum or collapse of the nasal valve. I would recommend seeing an experienced board-certified surgeon who has experience with functional septo-rhinoplasty.

This is a difficult question as there are many factors as to why you may not be able to breathe through your nose. Some of the more common reasons include deviated septum, nasal valve collapse (narrow nose on the inside), enlarged turbinates. Insurance companies will cover certain surgeries with the nose depending on the source of the nasal obstruction. I would recommend you see a board-certified surgeon experienced in dealing with functional as well as cosmetic rhinoplasty.

Should I get a septoplasty? (Photo)

Septoplasty is a procedure to correct a deviated septum. It can be performed through a closed or an open approach. In order to tell if you have a deviated septum an internal exam of the nose needs to be performed with a small nasal speculum. Other issues that can cause nasal airway obstruction such as nasal valve collapse can be assessed at that time as well.

Usually a septoplasty is unlikely to change a droopy nose unless it involves the very front end of the septum. Usually treatment of the droopy nose involves a rhinoplasty approach.

There are multiple possibilities as to why you are having nasal airway obstruction after a septoplasty. Potential things could be collapse of the nasal valve (the cartilages that make up the tip of your nose), large inferior turbinates, underlying medical issues such as allergic rhinitis or sinusitis. I would recommend seeing an experienced fellowship trained facial plastic surgeon with an otolaryngology (ear, nose, throat) background to help evaluate you.

I would recommend you get examined by an Ear Nose Throat Surgeon (otolaryngologist) or Facial Plastic Surgeon as soon as possible as you may have a collection of blood or a hematoma in your septum and /or nasal or septal fracture. Good luck and all the best.

While the medical conditions you mentioned don't seem to be contraindications to undergoing a general anesthesia I would consult with your surgeon and your anesthesiologist pre-operatively to discuss your concerns so they can go over the risks and benefits in your particular case. Good luck!

Before you have a facelift I would consult with an otolaryngologist and/or neurologist to rule out the etiology of the weakness. If they believe it is truly a Bell's Palsy (in other words they ruled out any other treatable or reversible causes) you would be an appropriate candidate for a facelift. Usually the procedure can help with some of the asymmetry at rest, but will not change the weakness when smiling or any dynamic motion of your face. I would see an experienced facial plastic/ plastic surgeon who has experience in dealing with patients with facial paralysis as well as facelifts. All the best.

New Confidence and a Working Nose!!!

I am writing this review a little over a week post op from a Septoplasty/Rhinoplasty/Turbinate Reduction surgery. Dr. Desai and his team are incredibly talented and professional. From the initial consultation to my post operative appointment, Dr. Desai made patient care his priority. His expertise and attention to detail paid off; not only can I breathe properly through my nose, but I have a new found confidence! The cosmetic alterations were done exactly how I envisioned. I finally can look in the mirror and feel like myself again (I’ve been dreaming of this surgery for over 10 years!!) Lastly, the entire process was easy and reasonably priced, the hospital nurses and staff were nothing but helpful and professional, and I would absolutely recommend anyone to see Dr. Desai!

Gamechanging Experience

I broke my nose playing basketball in the second grade. The ER doctor reset the nose as best as he could, but I have had nasal breathing issues ever since. As I grew older, I just got used to it. I never fully realized the extent of the problem; I just figured that's how it was. Two months ago, I finally worked up the courage to go see an ENT and was subsequently referred to Dr. Desai and his team. A real life example of serendipity. At the initial consultation, Dr. Desai inspected my nose and diagnosed me with a deviated septum, enlarged turbinates, and nasal valve collapse. He explained each condition and outlined the options for moving forward. I also want to mention that I was reffered by another doctor in his office who I originally had the last appointment of the day on Friday, so Dr. Desai was sticking around late on a Friday to talk with me. So nice and greatly appreciated. I ended up waiting a month before I scheduled a septoplasty, turbinate reduction, and spreader grafts. The surgery was on a Wednesday morning at the outpatient center in the same building as Dr. Desai's office. I won't bore anyone with the specifics of the surgery or the recovery since this is a review of Dr. Desai and not the medical procedures themselves. Just know that everything they tell you is honest and true. Dr. Desai knows what he is doing. I am eight days out of surgery and I have never breathed so well in my entire life. The rest of his team was just as fantastic. They were knowledgeable, kind, professional, considerate, and empathetic. Special shout out to Sophie for being particularly awesome. She answered my numerous e-mail questions with responses in minutes. I could not have had a better experience with Dr. Desai and his team. If you are considering any of the procedures that are in his area of expertise, I highly recommend that you see him. You will not do any better than this group - true professionals.

I spent a very long time having issues breathing through my nose and met with Dr. Shaun Desai hoping he would be able to correct the issue. He was very attentive and thorough in his examination and within moments Dr. Desai knew what the problem was and recommended a path forward. On 21 March I had Rhinoplasty and Septoplasty surgery at Suburban Hospital. Suburban Hospital’s staff is very friendly, courteous and I had minimal wait time from check-in to surgery. Prior to surgery, Dr. Desai visited with my husband and me, making sure we had no concerns or unanswered questions. Post-surgery Dr. Desai continues to monitor my health, ensuring I’m healing well with no issues or complications. He is always very professional, patient and compassionate. I recently had a Rhinoplasty surgery done at Johns Hopkins outpatient center. My doctor was Dr. Shaun Desai. I met with Dr. Desai twice before deciding to go forth with the surgery. During consultations, Dr. Desai patiently answered all my questions and explained thoroughly the surgery procedures and benefits. He is very professional, informative, and very compassionate to what I had gone through. I never felt rushed in my appointments with him. On the day of surgery, Dr. Desai came to the nurses' station to make sure that I was comfortable and if I had any further questions before the surgery. The front desk staff is friendly and courteous with no excessive waiting time. The nurses are excellent, caring and efficient. Overall, I had a very good experience. I can breathe much easier now.

He gave me the necessary prescriptions post surgery to get back up and running again. Very professional and I wouldn't recommend any other doctor for such a procedure. I'm 35 and over the last few years, I’ve noticed those fine lines on my forehead had become more obvious and I have those annoying 11 lines. I really wanted to find a surgeon in the DC-bethesda area, near where I work. I was very pleased to see that there was a Johns Hopkins facial plastic surgeon in the area. I met with Dr. Desai and was immediately impressed. His office is convenient to where I work, the staff are very pleasant, and he is great! I was really concerned about looking too frozen, but Dr. Desai did a fantastic job. He really understood what I was trying to achieve and made great recommendations. He performed a Botox treatment to my forehead and crows feet. It was painless with no bruising! I was able to go to work that same day without anyone noticing. I would highly recommend Dr. Desai, he is skilled and thoughtful. I intend to make another appointment for filler soon.
