She is my fabuloso doctor! Dr. Kapoor found a suspicious tumor that was growing on my lid. It was really ugly and bothersome. I went to Dr Kapoor to see if it could be removed and she did it on the same day! She made it painless and was so detail oriented that I don't have a scar or even a divet. Crazy! Hi beckaroo1, thank you for your excellent question. Juvéderm and Restylane, which are hyaluronic acid based fillers, are excellent choices to be used peri-orally. The oral commissures (corners of the mouth) are strategically targeted with fillers to bring up drooping corners of the mouth. Also product can be placed in the area of marionette lines to offer additional support in the area. Interestingly, if you have any midface volume loss, using Voluma (a more viscous filler with "lifting" ability) in the cheeks can also help bring up the corners of the month, in addition to softening nasolabial folds. I would also consider a neurotoxin (e.g. Botox) which can be placed in the lip depressor muscles to help relax and lift the mouth. A synergistic approach is best! Best of luck, Dr. K
Hi Lovelygrace thank you for the great question. Melasma is a very tricky skin condition that tends to hyper-react to laser and light. Although the Clear and Brilliant laser (particularly the 1927 nm Permea Handpiece) can be recommended for Epidermal Melasma, the risk of worsening is still there. Thus it is highly recommended to precondition the skin, by calming the pigment-producing cells (melanocyte suppression) with a hydroquinone or kojic acid based bleaching cream for 4 to 6 weeks, prior to starting the Clear and Brilliant laser series. In the meanwhile please contact your physician and let him or her know of your rebound Melasma. In the interim, it is imperative you avoid the sun and to always use a high SPF (>45) broadband sunscreen, as UV rays can also contribute to the worsening of this skin disease. Best of luck, DR. K

I definitely recommend contracting your treating physician for these details, as every practice generally will have a customized, written set of pre and post care instructions you will follow for your procedure. Note: Every practice will differ in policy on when to stop a topical irritant before a procedure and when to resume. Thanks Moonjava for the terrific question. Eyelid skin is very thin in general and can be one of the first places that begins aging. This can lead to fine lines and crepiness, in addition to the appearance of dark circles (either from pigment or underlying vasculature). A great way to "thicken" the skin is to use a combination of one or more lasers (fractional, non ablative) or radiofrquency devices which can help the body rebuild its own intrinsic collagen. This takes time, over a period of months. These devices can be used on the body as well. Adding filler under the eye as well as neurotoxin (BOTOX) to relax muscle causing dynamic wrinkles will synergistically rejuvenate periorbital tissue. Be sure to consult a Board Certified physician in any of the core aesthetic specialties (Ophthalmologist/Oculoplastic Surgeon, Plastic Surgeon, or Derm) who has had a lot of experience in the field to assure you will get a great result. Aging can come about as a result of intrinsic (eg, genetic) and extrinsic (eg, environmental) factors, resulting in deterioration of skin (spots, fine lines and wrinkles), deflation (volume loss from diminishing collagen and elastin) and descent (gravitational forces) of tissue. Over time, this causes the youthful upside down triangle to become an upright sagging triangle. A great rejuvenation and preventative aging program best attacks all these aspects of aging to help transform one's appearance: 1) a. Lasers (fractional, ablative or nonablative) and IPL (Intense Pulse Light) can improve fine static lines, pores and color (reds, browns) aberrations. b. Neurotoxin (eg BOTOX) can help mitigate dynamic wrinkles which can slowly etch into the skin. You are correct..the sooner you are able to stop the movement, the easier you are able to soften those etched lines. Stubborn lines can be improved with lasers. 2) Loss of volume can be addressed with fillers. A less viscous product can be used for nasolabial folds, while a heavier product can help volumize cheeks. A product with lifting ability, such as Voluma, can improve sagging tissue while volumizing. 3) Modalities to further improve descent of tissues include devices which slowly help the body build collagen such as radiofrquency (Thermage, Profound, Sublime) or ultrasound (Ultherapy).
