Dr. Chapin & his staff are amazing. I am an OR nurse with years of experience working in plastic surgery. My first year in the field, I unfortunately had a botched breast augmentation. In addition to being aesthetically displeased with my breasts, I was in a car accident that left me with bilateral capsular contractures and constant pain. Knowing revision surgery would be challenging, I was incredibly picky and apprehensive while I researched physicians that specialize in breast revisions. I went to many consultations and Dr. Chapin was by far the most thorough, confident, honest, and knowledgeable doctor I had visited. I am now nearly 4 months post-op and beyond happy with my results. Dr. Chapin and his team are awesome! Everyone in his office was very friendly and accommodating. I had a breast reduction and went from a 34DDD+ to a 36D. I could not be happier with the results and can now wear a bikini without looking inappropriate! I am 29 years old, 5' 2" and hover around 135-140 lbs. Dr. Chapin was able to remove about 350 cc's from each breast which was about a pound and a half total. Consult: I had my free consult, in September 2019, where I felt very comfortable with Dr. Chapin. So much so that I decided to go ahead with getting the necessary letters to proceed without exploring any other doctors. He did not give me much information about the surgery at this point because I needed to wait to see if I was approved. I was also given a figure for how much it would be if I did not get approved. (around 7k) I was able to get letters of recommendation from my Obgyn, my primary care physician, and my co-worker who has treated me a few times for upper back/neck issues (due to chest). I should also add that I am a physical therapist so I have self-treated for years and made sure they all included that in their letters. What prompted me to get this surgery was my inability to get into the best positions to treat patients without my chest in the way. I also began getting intermittent tingling in both of my pinkies due to the bra straps pinching down. With the 3 letters and Dr. Chapin's examination, I heard back from my insurance company about a month later and they approved the surgery. I have UnitedHealthcare and I believe they also refer to the Schnur chart when making their decisions. It essentially uses your height and weight to determine the amount of breast tissue that would need to be removed to make it "medically necessary". After I was approved, they called me and I set up the date for my surgery! Next was to wait until a few weeks before the surgery. Pre-Surgical Visit: I went back to Chapin Aesthetics in mid December. This is where I got ALL my information and questions answered. I brought along my boyfriend for this visit so I had a second set of ears. I asked about the type of surgical procedure used (only because I found a research article about how technique will play a part in whether you can breast feed down the road) and learned that Dr. Chapin uses the recommended method that will still give you a fair chance at breast feeding later on. One of his nurses went through all the pre-operative paper work with me and also all the prescriptions that I had to fill. There were about 5 of them: Emend (to take prior to surgery to help with nausea from anesthesia), Cednifir (an antibiotic), Celebrex (for inflammation), Gabapentin (for nerve pain), Oxycodone (for break through pain), and there was another for nausea but I never ended up using that. I was also advised to get extra strength tylenol. Prepping for Surgery: I made sure to order some front zip sports bras to wear post-surgical. I also ordered a big foam wedge for my bed (since its most comfortable to lay face up for awhile). I ended up having to buy some gauze pads later on down the road so that would have been helpful to have post-op. I also was fully prepared to have my boyfriend with me for at least 2 full days after surgery. I also had to buy a special soap (hibiclens) to wash the chest area with for 2 days prior to surgery. Pre-Op Testing: I had to go do pre-op testing at Doylestown Hospital (where I was to have the surgery) a little less than a week before surgery. Since I am relatively young and healthy- they only needed one vial of blood and a urine sample. Even though I had an IUD, I still had to get that urine sample to make sure I wasn't pregnant! Surgery: I had my surgery on January 6, 2020 at Doylestown Hospital. I was called the day before with my surgery time which was 11. My boyfriend came with me since he was going to be driving me home. I was taken back to my "room" which was just a curtained off bed in what to me looked like the short procedure holding room of patients. I had a nurse's aide guide me back and had me get into my robe. I had to use 2 big wipes to wipe all along my chest, given to me by the nurse. A nurse came back, who was very friendly, and we went through all the medications I am on and what I was instructed to take. She put my IV line in and then soon after Dr. Chapin came in and greeted me. Actually he had the best joke and said "So how big are we going again?" and then laughed and said, "No I'm joking", and that actually helped me relax a little! (haha) He then marked me all up which took about 15 minutes. At one point the anesthesiologist came back and also spoke with me. We went over my medical history and I noted that I do take marijuana for medical purposes. I later read an article that you should always tell the anesthesiologist if you use marijuana because it can effect the dosage needed for you! ALWAYS TELL THEM. I was then taken back to the surgical room, got a nice IV of high dose valium, and then woke up what seemed like 30 minutes later! In reality, the surgery was almost 2 hours long and I came to in the recovery room with a nurse right next to me. Basically as soon as I opened my eyes she asked how my pain was. I honestly was pretty numb so did not feel much. She had to open up the surgical bra to check the incisions and I was still pretty loopy. I did have some difficulty with nausea right after surgery. She gave me an injection in the thigh to help with it. It seemed to happen when I tried to sit upright and I think it was more due to fluid loss and sitting upright made me feel faint. When I was moved back to "the holding room", I had to be wheeled over to the bathroom since sitting up was making me feel ill. Once I felt better, the nurse was extremely nice about quickly giving my boyfriend any instructions, some extra gauze, a cold wet wash cloth for my forehead and had him pull the car around while a volunteer wheeled me to the entrance. I had to lay back the whole way home in the car and when I got home I got into bed on my wedge pillow and slept for a good 2 or more hours. Recovery: The first 2 days I definitely needed my boyfriend around as it was very hard to reach out to the side or reach up and I had some discomfort bending forward. Otherwise, I was pretty numb. Occasionally I would get some increased pain and have to take the pain medicine but for the most part I did well with motrin and tylenol. I took 2 weeks off from patient care at work and was still not treating certain patients for 4 weeks following surgery. I was able to answer e-mails and do some admin work from home 2 days after surgery. Dr. Chapin had me send videos of capillary refill of each nipple through the secure Symplast app the day after surgery. Sending these videos to the nurse saved me a trip to the office on post-op day 1. The first week after surgery is really just laying or sitting, putting gauze pads in my bras, and trying to assume a normal posture. During the second week I had very severe upper back pain due to slumping forward from the tightness of the incisions. I regained normal sensation in my right breast relatively early, but my left remained pretty numb for at least 3 weeks. Certain junction points of the incisions had oozed slightly for weeks and weeks which was to be expected per Dr. Chapin. I was back at work at this point and was still having some oozing so gauze was my friend. I also had a small spot on my left nipple that would not properly heal. On one of my post-op visits, Dr. Chapin cut it open a little more along the edge since it was tunneling. A little more gauze and some neosporin later, the wound healed up after 2 weeks. At about 4-5 weeks post-op, I was dealing with some extreme itching around the incisions and it started to look like a rash. I was given a steroid pack to take and had a very horrible emotional reaction. The steroid pack made me into a crazy person. I have taken steroid packs before and never had this response! But I will now never take them again. Although the steroid did help with the itchiness. Lotion and ice helped also. I was able to exercise and begin running again at about 8 weeks. I did still have some scabbing and healing though still going on at this time. Now I am almost 4 months post-op and I am very happy with my results. They are perfectly shaped and I have normal sensation in both breasts. I am able to lay on my stomach without any issues and I have had no upper back pain! I received a breast reduction from Dr. Chapin in November 2019, and it was the best decision I've ever made. I consulted with him in July 2019 after a terrible consultation with another surgeon. The first surgeon was abrasive and cold, and Dr. Chapin was the complete opposite. His professionalism, knowledge, compassion and sense of humor made my decision easy. I felt very comfortable and "heard". From initial consultation to post-op, I felt at ease and never anxious, which is huge for me. Post-op communication and bedside manner have been fantastic and easy, as the practice uses an app which allows you to send questions, photos and videos directly to Dr. Chapin and the nurses at any time of the day. That has been extremely helpful for me, as I live an hour away from the practice. I would reccomend Dr. Chapin and his team to anyone interested in the same or similar procedure. He is truly an artist.
I waited over a year to post my review because final results are not achieved in three to six months. With that said, the mini lift accomplished exactly what I desired. I really needed to have my neck done, but to keep cost down I chose to just do the mini lift. Although I kinda regret it, it's not enough to dampen my happiness over the positive results. Very natural result. Got rid of jowls, although i still have a bit of a double chin; I was advised to have that done as well. I'll do that another time. No pics sorry. Not even sure my results would be all that discernable in photos, although to me it's a mammoth improvement. I do feel like that subtle difference took years off my face.

Thank you for your question. I know you mentioned you would prefer not to opt for a breast lift. However, if you move forward with a breast augmentation without a breast lift, you will have the same or even more significant drooping of your breasts post-procedure. Currently, your breast tissue and nipple fall below your inframammary fold. If an implant was placed in the breast pocket without a lift this would cause your breasts to drop even lower. I think you would be extremely dissatisfied with your end result without a breast lift. I recommend you schedule an appointment with a board-certified plastic surgeon who specializes in breast surgeries for a consultation.
One position is not better than the other. Choosing which one depends on several factors including the patient's skin elasticity, amount of breast tissue and aesthetic goals. Typically speaking, under the muscle gives a more natural look and hides the edges of the implant nicely, particularly in those who don’t have an abundance of breast tissue. When placed over the muscle, the implant is more visible and gives a fuller, rounder appearance. I recommend clearly communicating your aesthetic goals to a board-certified plastic surgeon, and they can determine the best implant placement.
Thank you for your question and for sharing your photos. Two months is too early in the healing process to determine the implant position. It is not uncommon for one implant to settle more quickly than the other. Typically, it takes six to eight months for implants to completely settle. The worst-case scenario is, if it doesn't settle, your surgeon may need to manually pull the implant down slightly. It is important to stay in communication with your surgeon during your recovery and to follow his recommendations.
Based on your photos, you would be a perfect candidate for a breast lift and breast augmentation. A breast lift would reposition your breasts for a perkier, shapelier appearance, and a breast augmentation would add more fullness to your breasts.
