To answer your initial question --- yes, a tip rhinoplasty could be performed without addressing the dorsal hump. However, to answer your second question, I think the best way to proceed would be to have a consultation with a facial plastic surgeon to go over the options or ways to soften the nose. Most facial plastic surgeons will have image morphing software where they can take either a 2-D or 3-D image of your face and morph your nose to simulate various maneuvers (i.e. tip rhinoplasty +/- dorsal hump reduction). Sometimes it can be helpful to get a copy of these morphed images to take home and think about the potential cosmetic changes and/or also get the opinion of family and friends. Finally, if you are thinking of moving forward with cosmetic nasal surgery, it would also be a good time to address any functional nasal concerns such as the deviated septum, which your facial plastic surgeon will also be able to correct.
