Porn Star Look is yours with 700cc or 800cc over the muscle.  They will not be freakishly huge but large just the way you imagined.  I agree with Dr. Placik regarding talking to your surgeon about putting them sub-muscular for 3 reasons

1. More natural looking
2. reduces capsular contracture
3. aids the technician when performing a mammography

450-475 cc implants may look just right if the implants are high profile.  However you may be able to go slightly smaller with high profile.  High profile implants are smaller in circumference and provide slightly more projection forward.  There is also an Allergan Style 45 which looks great on a body such as yours with a small chest.  There may also be a similar style of implant with the Mentor and Silimed companies.   Your plastic surgeon may have the implant sizing chart so that you can see the different dimensions.  Of course the best way to know is to try on sizers and listen to your surgeons suggestions.  Then think about what makes sense for you.  I hope this is helpful to you.

It is really too soon to know the finished outcome of your recent breast augmentation.  You ought to look at your before photos to see if the left breast was lower at the crease than the right breast.  Also, look at yourself in the mirror and stand as naturally as you do without thinking about it.  Are your shoulders even?  Or, there may have been more bleeding on the left side and it will take longer to re-absorb.  The body reacts asymmetrically during surgery and in the healing stage.  With over 40 years of performing breast augmentation you need to know that I have seen some patients take over a year for the breast to finish the healing process.  Certainly at 3-6 months the outcome is close to what they will look like at a year.  Patience in letting the body heal is so important.  We all want the outcome right away.  Time is on your side.

Choosing an implant size that is a little larger (think around 400-450cc or more) in size may give you the look you want.  With your size of breasts and the nipple position, why don't you get the implants, let them settle for 1 year and then decide if you want the nipples higher.  Under the muscle is the very best choice for many reasons.  First, they will look more natural with a muscle covering, less rippling, and more protection for the implant.  Hope this was helpful and as always be sure your surgeon is boarad certified to do plastic surgery.  Board Certified Cosmetic Surgeon is NOT a legitimate board.
