Richard is an excellent Doctor and very experienced. I am very particular about who works on me and I cannot recommend highly enough! Over many years he has treated me with Botox, lip filler (and I have tricky lips!), facial filler and threads and it has all been brilliant. He is very honest and direct and will not treat if he feels it will not benefit.
Lip filler can be used to enhance the volume and widen the top lip. There is a limit on how much improvement you will get and you are unlikely to get the result shown in the other picture as your natural anatomy is very different. The lip filler will fill the volume especially on the outside edges, but there will always be a tendency for the top lip to turn in at the outside edge, which will limit the length achieved. If you want perfection or close to the pictures you shown, then this is going to be achieved, so I wouldn't recommend the treatment. If you would be happy with some improvement then you would get some benefit from the treatment.
At the doses you describe the botox would usually last about 3 months, sometimes a bit longer. Once the Botox has warn off it may take time for the muscles to build back up again. By encouraging the muscles to work, you may speed this up a little, but the symptoms that were being treated are merely to return quicker. Dermal filler can be used to fill the hollows, but I wouldn't recommend this if you don't plan on having the botox again. Hope this works out.
Lip filler can be done with a canula, which is less likely to injure blood vessels, this is passed along the lips from the side. My recommendation would be to get a consultation with an experienced Doctor so they can examine and palpate the lips to assess where the blood vessels are. They can then advise what the best approach would be and the likely results for you. There would be a risk of bruising which may be significant, as there is always a slight risk of problems even when using a canula.
Dermal Filler can be place along the chin to bring the chin forward slightly. This uses the thicker products which can last about a year. There is a limit to how much lift can be achieved which would be discussed a consultation, along with any risks. I would recommend getting a consultation with an experienced doctor, so they can advise you on likely outcome so you can decide if it's something you want to do.
HA fillers normally last about 6 - 12 months. I have seen some people where the filler last longer, with no problems, even up to 3 years. If there are no issues with the filler, just leave it alone and at some point it is likely to dissolve. Occasionally there is a small part of the filler that seems to last, with the rest breaking down, which is probably due to a capsule around the filler. If this is causing a problem it can be broken down with Hyalase. Rarely there is a thickening of the tissue, rather than residual filler, which may be more difficult to break down. My recommendation would be to get a review with your Doctor, so that you can both decide what is best for you.
