There is some confusion and misunderstanding between terminology, and there is probably disagreement. As an experienced face and neck lift surgeon, I modify my approach and procedure upon the findings when I see the patient. Usually a surgeon will recommend lifting the neck via a face lift approach combined with a direct neck approach, to both suture the decussated platysma together anterior but lengthen posterior lumbar as well as remove fat if needed. Rarely in a younger patient with an isolated fatter neck Vaser or Lipo can be performed either alone or with an isolated anterior surgical approach, but this is in a small proportion of cases. En e the key is to see a plastic surgeon with the correct certification but more importantly expertise in face and neck surgery in particular.
Seven days is way to early to do anything. You best to wait at least three months for your swelling to settle before any conclusions can be made. I always advise a tennis headband at night for three months to reduce the chance of malposition when sleeping.
In your case the most important issue is that you have capsular contracture, so you need to ask your surgeon to try to reduce the risk or this recurring by using polyurethane implants. These should match your breast width. You cannot really predict what your breasts will look lie exactly due to the fact that you have not only having older implants updated but you are having capsule removed.
Generally speaking three weeks after rhinoplasty is far to soon to make any judgements, I would be patient and see your surgeon in due course.
Scars after this procedure, and so soon after surgery are common, you have forces in two opposing directions. I would trust your surgeon and carefully follow his advice.
