Good Morning. Dermal fillers typically don't leave any superficial scarring. Your are in question looks like either acne or eczema. Though it's difficult to tell without examination. Rarely, someone forms a granuloma ( an organized collection of inflammatory cells ). Again, it is a clinical diagnosis based on history and physical examination. Typically any RF treatments are superficial treatments that stimulate collagen formation. They should not affect HA fillers. However, I still encourage my patients to wait 2 weeks between any injectable and any energy treatment. Good luck Dr. G
Due to the nature of filler, it is possible to migrate slightly outside of the injection area. Though I find it typically is incorrectly placed filler. From you photo, I feel you need to dissolve the filler with hyaluronidase and start fresh. It appears the filler may have been placed incorrectly and possibly too generous in the upper lip. There is no interaction between botox and methotrexate. However, caution should be used when an immunosuppressant is used with any cosmetic injectable. Besides filler, you options are lasers skin resurfacing, radio frequency treatments or microneedling. I hope that helps. Good luck. Dr. G
