You will need a new bridge made to replace the extracted teeth. Fix prosthetic like bridges are made to fit the surrounding teeth and tissues perfectly.
Most dental insurance companies have a 3 to 4 week turn around time for pre-treatment authorizations. Be sure to go over the pre-pretreatment authorization and understand what portion of the overall treatment is covered by your insurance company and what is the portion you will have to pay out of pocket. Typically, insurance companies will pay for only a portion of the overall treatment.
Most laboratories take about two weeks to fabricate a crown after they get the impression. It all depends on the specific laboratory as the previous DDS mentioned. It may also depend on the availability/schedule of the Dentist.
Sorry to hear about your situation. I would make a trip back to your dentist for an emergency appointment and have the area looked at. It may be the tooth (#19) or could be another tooth that is causing what you are describing.
Generally speaking, when you are getting "fitted" for a crown, an adjacent crown should not come loose unless there are other problems associated with it not related to "fitting" for a crown. ie There was a fracture.
