Placing implants after a Laforte procedure should be fine as a Laforte moves the entire jaw forward as one piece. You need to make sure that there is enough bone in the area where the implant is being placed. Have you checked out other options for advancing your upper jaw other than LaForte(surgery)? Try to find a dentist that is trained in controlled arch orthodontics where they use appliances (ie. AGGA- anterior guided growth appliances). This may allow you to avoid a rather invasive procedure.
It is really hard to tell with the limited photos but there is rarely ever a case that premolar extraction should be advised. More space can be achieved(predictably) with appliances so that teeth extractions are not necessary.
See if the situation improves as often it does take a few weeks for things to feel normal. If the problem continues then suggest to your dentist to have the crown replaced with a new one.
It sounds like your periodontist is doing the right thing at this time. He/She will be able to get a little more information as to why you are having discomfort still. If the bone graft is causing the problem then it can be removed at the same visit.
