I had a breast lift done by Dr. Afridi and I am so incredibly pleased with the result. I struggled with prematurely sagging breasts starting in my teens and it became a huge insecurity for me. I am so grateful for Dr. Afridi and her stellar team! I’ve had a lifelong insecurity over the size, shape, and overall appearance of my breasts. I am also tall with a natural hourglass shape, so I always felt like my breasts were not in proportion to my overall figure. I often taped my myself to achieve cleavage, stuffed my bras, wore uncomfortable padded push-ups, and generally did all sorts of smoke-and-mirrors tricks to adequately fill out dresses and tops. Hi Alex.Your pictures are very helpful and give a good sense of your situation. Overall I observe that you have stretch marks in the upper breast with large areolae (the brownish red skin surrounding the nipples), high tucked in inframammary folds and concave upper breast poles. Generally in patients such as yourself there is a history of never having ideal shaped breasts since their development in keeping with a diagnosis of mild tuberous breast deformity - high tucked in folds and large areolae. Also many times the stretch marks and the concave upper poles indicate either substantial weight fluctuations or pregnancy and breast feeding. Honestly, in my practice I almost never use implants as large as 450-500 and if I would even remotely consider it, it would not be in someone with as much baseline breast volume as you currently have. Very often surgeons attempt to improve breast droop and low nipples with a large volume augmentation. With more data and experience, a lot of plastic surgeons are now staying away from this philosophy. The reason being this may temporarily give you a reasonable result but will have terrible long term complications including stretching of the skin and destruction of your breast parenchyma. Very often patients develop worse drooping in the long term with bottoming out and unsightly breasts. When I have a patient such as yourself present to my office, we will usually consider a much more modest augmentation from 250 to 350 max. Also I think most likely the combination of this with a periareolar (circle around the areola) mastopexy should give a nice predictable result. The periareolar mastopexy will make the areolas smaller and will also take care of the elongated and "tuber" shaped breast. As far as choice of implant, this is individual and should be decided with your board certified plastic surgeon. A simple smooth silicone in the sub pectoral pocket should work alright but some would say under the gland/over the muscle can be useful. Others might consider an anatomical shaped implant. Really any of these can work well and it depends really on the surgeon's preference combined with the patient's input and what has worked in his/her hands in the past.
This is a great question and one that can be very confusing for patients. I am sure each surgeon is different so I can really only comment on my personal approach to breast augmentation. Firstly, I like to get a sense of the patient and her expectations. Patients presenting for breast augmentation run the spectrum of cultures, body types, professional interests and values. It is important for the surgeon to understand what the patient wants and what she envisions as beautiful breasts. Is she interested in a very natural looking breast or is she attracted to a breast with a lot of upper pole fullness and roundedness? The latter is really not natural looking but has become a commonly sought after look heavily displayed in the media. This information strongly impacts which profile of implant will work for the patient. First of all, profile just means how much projection the implant has when you lay it flat against a surface. So if you take for example a 300cc implant in all the different profiles; it will be quite wide and flat if it is low profile and very round and projecting if it is high or ultra high profile. The way this decision is made in my practice is that my patient and I "try on the look" and determine what volume (cc) of augmentation the patient likes. If she likes how she looks with a 300cc sizer then I measure her breast dimensions and determine what profile would fit her breast base but still give her a 300cc volume increase. I would say the majority of implants used in my practice are moderate and high profile implants. Only a patient with a very wide breast or one who does not want significant augmentation would be suited for a low profile implant. In the same vein, I am very cautious with ultra high profile as these are really "in your face" implants and it takes a certain strength of conviction and attitude to carry these. First of all, with the three surgeries you describe, I would suggest splitting these into two separate surgical days. These can be as close as 2-3 weeks apart if you recover perfectly from the first surgery and you and your surgeon plan correctly. Clearly, the liposuction must be combined with the fat transfer to the buttocks. I would consider this one day of surgery especially if there are multiple areas of liposuction or harvest of fat. This surgery always ends with you laying face down in order to create and shape the buttock. It is best then to stay off the buttocks ideally for about 10 days minimum. This becomes a problem if the breasts are done in the same session due to prolonging the surgery but also compromising the breast surgery by laying on the breasts to protect the buttocks. The breast lift should therefore ideally be done as a separate procedure. As far as deciding which to do first, I would be inclined to say do the lipo and buttocks first. This will allow you to follow up with the breast surgery more quickly since you can lay on your buttocks for the breast surgery anytime after 2 weeks. If you do the breasts first then I would not recommend doing the second surgery for generally 4 weeks as the breasts need to heal and recover. However, the downtime from the lipo and buttock augmentation is longer than the breast lift so if your work or school has intervals that are more favorable for doing the lipo and bottom first then that is the way to go.
