Thank you for your question and I'm sorry you're having a difficult time. Your best bet is to review this with your surgeon, as they know best what was done during your surgery. Without prior photos and knowing what was done (like from a prior operative report) and examining your nose, it's difficult to give you any specific advice. 

You do have a supratip fullness. In some cases that can be addressed with just steroid injection. Your tip is also overly rotated. And the columella is healing poorly - bad scar and distortion of the medial footplates. This can be repaired, but the exact approach will need to take into account the state of the septum, and additional grafts may be needed. 

The fact that you did not have splints does not mean anything. During septoplasty, I usually put stitches in the septum to avoid stents whenever possible. 

Discuss your concerns with your doctor, and if not satisfied, seek second opinion from someone experienced with revision Rhinoplasty.

Thank you for your question and I'm sorry you're having issues. Discuss this with your physician first, as they know your nose and what was done better than anyone else, and may have additional suggestions for you. 

For people who have chronic nasal problems, I like to start out with flonase and saline rinses (like Netty pot) as the basic medical therapy. And these may be necessary even after a perfect surgery. 

If your surgeon is unable to address your concerns, seek a second opinion - preferably from a facial plastic surgeon.

Revision rhinoplasty is one of the most challenging surgeries to perform. Find an experienced surgeon you trust. Only after a full and proper exam, can a personalized plan be developed. I expect you will need significant amount of cartilage grafting to reshape and support your nose. Having had a prior surgery, the amount of cartilage available from your nose is likely to be very limited. This will likely mean having to borrow cartilage either from the ear or from a rib. This is fairly standard practice.

Also, nobody's face is symmetric.  In your case, the right side of your face is the smaller one. This needs to be taken into account when making the surgical plan for your nose.

It looks like there's still quite a bit of swelling - like your surgery was very recent. I recommend all my patients to wait at least a year before making adjustments to their rhinoplasty. Discuss your concerns with your surgeon, as sometimes steroid injections may be indicated/helpful.

This graft is usually sutured in place during initial placement. It is certainly possible to elevate the skin without disturbing it. I recommend you discuss this question with your surgeon, as he's the only one who knows what maneuvers were done and whether the  graft was disturbed, removed, replaced, or left alone.
