I had an excellent experience with Dr. Bassiri! I went into the consultation with him not knowing exactly what I wanted, just knowing what I didn't like. He listened to me and was able to understand and articulate back to me the results I imagined and how to get there. My experience with Dr. Bassiri and his office has been such breathe of fresh air. He did my rhinoplasty surgery about 2 months ago and I truly don’t think I could be any happier with the results. From the start when I went in for my consultation I knew I was in good hands. Brittany, his surgical coordinator, was so friendly, responsive, and warm while setting up the appointment and greeting me in the office. I met with Dr. Bassiri for over an hour talking through everything, doing photos and morphs, and asking questions. He completely understood my vision and the importance of a natural look and still looking like “me.” I never once felt rushed or pressured and just got a great feeling from that initial appointment. I had planned to meet with a few other surgeons but canceled because I knew I was going to go with him. Leading up to surgery him and Brittany were super communicative, available, and friendly. I felt nervous before my surgery, which he completely validated and helped ease my concerns. He performed a closed rhinoplasty and everything went super well. My swelling peaked on day 3 after surgery and from there the swelling went down super quickly. I was sure to carefully follow all of his pre and post surgery guidelines, so recovery went very smoothly. On day 7 I did have a small bump in the road when I got a severe nose bleed, but he talked me through it on the phone and met me in his office early in the morning to help. We resolved that same day just with some dissolvable packing and there were no further issues. Since then, my nose has only gotten better and better. Mobility after surgery is a frequent question. On the first day after surgery, I place a snug head/neck dressing. This limits some of the mobility. After the first day it is removed and the head can be turned and moved normally. It is important to avoid sudden or extreme movements. The idea is to avoid undue tension. Neck muscles may feel sore after surgery so it is good to move the neck to avoid it getting stiff. By three weeks after surgery, my patients return to full activity, exercise, gym etc. Some patients think they have to move like a robot and turn their whole body rather than twist their neck to look in another direction. It is not necessary to do that, and other problems can arise, such as car accidents. Ie: if a patient is driving but doesn't want to move their neck to check mirrors properly (It's happened). So while I would avoid perfecting new yoga poses during healing, I would not avoid normal natural movements especially if done with caution.
Thanks for the question. Congrats on making the decision for surgery. Swelling like most things is patient dependent. There are measures that can be taken to help minimize it. As far as timing is concerned, most of the obvious swelling will dissipate by three months but it may take six months or more in some people (rare). There is a sweet spot at roughly at 4-6 weeks, where there is a minimal amount of swelling present , but it may look good and the skin can look plump and healthy eliminating residual fine lines. For this reason I advise patients to take four weeks before participating in a major social event. Factors that can help swelling after surgery include: lymphatic massage, ultrasound treatments, avoidance of alcohol and salty foods.
Like most things posted on this website, it is generally up to the surgeon to decide how long compression dressings are left on. Usually more extensive surgeries may require a slightly longer period of wearing a dressing. Typically, I place a tighter dressing at the end of surgery and remove it the next day. If I feel its needed for support, I will re-wrap the face/head for another day - this usually is not the case. I will regularly recommend the use of a more comfortable dressing to support the next and face for the next week. In my practice, we use something called a "face-bra". (this is not an ad). As the name suggests, it is made of 100% Lycra Spandex and is more comfortable than other dressings. I recommended wearing this as much as possible the first week and as much as comfortable at night for another week. Patients seem to like the support having some dressing provides. It also comes in leopard print.
As discussed in other posts, there are many variations on facelift techniques and names. The extent of dissection during surgery depends on the surgeon and what the patient needs. It is reasonable to perform the surgery completely awake or under complete general anesthesia. I have a dynamic discussion about this with all my patients. Some are scared of being awake and some are scared of anesthesia. Both have their risks and both are always done as safe as possible. My personal preference is a 'twilight' sedation that is in the middle. Here a patient is sleepy and comfortable, but arousable if necessary, much like a colonoscopy. Patients are breathing on their own. They are numb from local anesthesia injections, but are not awake feeling them go in. The board certified anesthesiologist makes sure all vital signs are stable and the patient is comfortable. This allows me to completely focus on surgery. When the surgery is over, many of the issues with nausea are avoided because anesthesia gas was not used. This technique is modified as needed for each patient, but is safe and has been working well for me personally.
Kybella works by disrupting the fat cells where it is injected. This causes a strong inflammatory response. The area of the neck warn, tender, and swollen soon after the injection. This swelling then persists in varying amounts for about 5-7 days. The swelling can be improved with some compression, like an ace wrap. In general, I advise patients to wear a scarf or collared shirt to hide this during the first week. While there is no physical restrictions with going about your day after injection, you may feel self conscious about swelling and may want to take the day off.
