When I was 19 I underwent breast augmentation. I was flat chested (not small breasted) and very self conscious of this at the time. After 12 years of living with the implants and about 2 years of consideration, I decided to have them removed. My priorities had shifted and I simply desired to return to my natural state. Though I had no conplications, I wanted to avoid the possibility of needing future surgeries. It’s been just over a month post op and I couldn’t be happier. Thanks to Dr. Keegan’s gentle hand and incredible skill, I had very little pain, minimal scarring and overall I am healing really well. He was also there to answer what seemed like a multitude of questions and responded to all my concerns along the way. I am really small now but was pleasantly surprised to discover that I now have more shape than I did prior to the initial surgery and my skin retracted nicely.
I would highly recommend Dr. Keegan. He took time to explain the procedures and made me feel comfortable. I trusted Dr. keegan all the way and I am very happy with the amazing final results. The office staff is also great.
Dr. Keegan and his team are amazing. They were supportive and helpful throughout, working around my schedule and needs. The results are amazing too. He is truly a gifted surgeon. Very little scarring - I don't know how he does it. And he takes all the time you want to explain every step. I am so happy I chose him and recommend him highly.
I cannot say enough great things about Dr. Keegan and SculpSure. Dr. Keegan is very thorough with great attention to detail. He has a way of making you feel very comfortable and takes the time to explain or answer any questions you may have. In addition, making an appointment with his office was a very pleasant experience. The nurses and staff follow suit to Dr. Keegan's example and overall the entire experience in the office was wonderful. I made the right choice in doctors and would definitely go back to Dr. Keegan and recommend him to others. SculpSure was also a great experience. The entire procedure took 25-30 mins per area. There was no pain at all. It felt like a heating pad on the affected area. After the procedure I had no bruising or discomfort. I was able to go about my normal life - returning to work and even going to the gym that evening.
Dr. Keegan is a superstar when it comes to plastic surgeons. I am so glad I chose him for my reconstruction surgeon. He is thorough, meticulous, kind and delivers beautiful results. I can rave about him for hours.

For the past year, I have been a breast cancer/reconstruction patient of Dr. Keegan at Fifth Avenue Millennium Aesthetic Surgery in NYC. Dr. Keegan was the 3rd plastic surgeon I consulted with, and I selected him as my surgeon based on his compassion and kindness. Dr. Port of Mount Sinai Hospital had recommended Dr. Keegan to me, so I knew that his surgical skills were impeccable as well. I live in NJ and I had to travel 1½ hours into NYC to see Dr. Keegan, and it was worth every mile (and GW Bridge crossing!). I knew that I was in for a long reconstructive process, so I selected the surgeon who I felt would “have my back” through it all. I wanted someone I could trust, have a rapport with, and ultimately have faith in his or her surgical skill to help me through this journey of cancer and breast reconstruction. For me, Dr. Keegan was the perfect choice to help me through this stressful period of my life. You see your doctor’s real surgical skills and emotional maturity come out when something goes wrong. My reconstruction journey was complicated by a staph infection in my left tissue expander (warning to all women undergoing breast reconstruction – BE CAREFUL SHAVING! Your underarms are not the same! Minor cuts can have major consequences.) When I noticed a bit of swelling in my left breast, Dr Keegan immediately put me on antibiotics and sent me for an ultrasound. He instructed me to have the ultrasound technician call him directly with my results, as he didn’t want to wait for the report. His instincts were right as I had a staph infection and he scheduled me for emergency surgery that evening. This was a terrifying scenario for me; however, Dr. Keegan’s reassuring words and manner helped me cope with a very frightening situation. He even held my hand during surgery until I fell asleep under anesthesia. Dr. Keegan was able to salvage my reconstruction and I continued on my journey with just a slight hiccup in timing. To look at my perfect symmetry now, you would never know I ever had an issue – again showcasing Dr. Keegan’s skill as a plastic surgeon. All journeys come to an end and I am happy and grateful to have had Dr. Keegan as my breast reconstruction surgeon. My results are outstanding and I feel lucky to have found him. I look beautiful and feel comfortable with my new implants. While the Fifth Avenue facility has much more of a tranquil, boutique spa feel, it has a certain “Cheers” quality. It was very pleasant to go where everyone knew my name. I always felt warm and welcome by all of the staff. Of special note are Chelsea – she was always “quiet perfection” in the examination rooms, and Julie – with her personable, cheerful demeanor – she always greeted me with a warm smile of recognition when I entered their offices. While I am glad that my trips into NYC are complete, I am delighted with my results! Updated on 8 Jan 2015: Please be gentle critics of my photography! Most of these are "selfies" which I shot either in my hospital bed or in my bathroom. I just want to point out that in my last photo - with my silicone implants - I had my mastectomy scars lasered 3 days ago (on Monday), so the scars look red and wide. In reality, the actual surgical lines are developing a lovely blurriness - and don't look like scars - especially along the center of the scar line. Updated on 10 Jul 2016: Hello all! I'm finally done with my reconstruction. Since my last post, Dr Keegan surgically created my nipples and Vinnie Myers did my nipple / areola tattoos. It takes a while to heal - Vinnie waits at least 6 months after the last surgery in order to give the scars time to fully heal. I posted several tattoo pics in different sizes and angles - so you would get an idea of the beauty and complexity of the tattoos. Vinnie really does fantastic nipple/areola tattoos. Feel free to zoom in if you can - he does some shading around the base of the nipple as well as those round dots on the areola which simulates the Montgomery tubercles (I had to look that up!) Vinnie sees a lot of breast reconstructions in his line of work - and he complimented Dr. Keegan's skill as a plastic surgeon. He said that my reconstruction is in the top 5% of the reconstructions he has seen. Vinnie said that he usually has trouble tattooing the reconstructed nipples because of the location of the scars the surgeon creates while recreating the nipple. Vinnie explained that Dr. Keegan creates nipples with the resulting tattoo in mind, so my result is a very natural looking breast. There is a wide geographical distance between the 2 men - NYC vs Finksburg, Maryland, and it surprised me when Vinnie clearly recognized Dr. Keegan's reconstructive work. I love my tattoos and they really do complete this chapter in my life. It's nice to look in the mirror and not be constantly reminded of cancer. It was ugly.
I was led to believe I would have pretty normal looking breasts. They have always looked horrible because of this one area. Dr. Keegan was my plastic surgeon immediately following my double mastectomy. I loved him However at an office visit shortly after surgery I was slowing pulling up my sports bra because the mastectomy was still fresh and the attending nurse got impatient and pushed up my elbow abruptly. The pain was incredible. That was 10 years ago. My doctors since then believe the muscle was torn and to this day I struggle with implants because that area huts and builds up scar tissue. T Finally after 4 redoes and thousand of dollars I'm having the implants removed and won't ever try again because the doctors keep promising they can fix the area but never do. I hope he fired that nurse by now. I can never prove the two are related but that same spot that I felt that incredible pain that day is the same spot that bothers me everyday..
His work, on me, a very young woman in college, a studious girl full of life and potential at Harvard, was grossly incompetent. He did not listen to me. He deformed me. He cost me my health. He diverted my life. He was extremely destructive.
Liposuction removes fat from discrete areas that are treated. The plastic surgeon must assess the patient pre-operatively to determine if the skin tone in the treatment area is adequate and will contract after the fat is removed. This leads to the improved contour and not skin laxity. If however, the skin tone is too loose, then the additional removal of underlying fat may cause inadequate skin contraction and some degree of relative skin laxity. This is not related to any particular area such as the flanks.Liposuction of the flanks can safely be performed alone or in conjunction with a tummy tuck.
I would say that a breast lift would not help your situation. You do have a relative excess of skin and a real loss of volume, but a mastopexy would not significantly improve things . Without the added volume of an implant or fat transfer, your options are limted
Personally, I do not routinely have patients wear a compression band. There are different nuances that each surgeon has to get the best outcome for their patients. Speak with your surgeon for their protocol
There are many potential factors which go into the risk of implant rippling post mastectomy and reconstruction. If you are thin and the mastectomy skin flaps were created equally thin, there is not much soft tissue to camouflage your implant. If you have a saline or a silicone gel implant, it is not unusual to see rippling at the upper pole of the breast and medically. If the rippling is very bothersome to you, consideration of conversion to a form stable, cohesive gel implant may give you some improvement. Autologous fat grafting may also be considered to camouflage the implant better.
It is always difficult to be dealing with such an open wound after surgery. If you indeed had a mastectomy, I would need to know what type of reconstruction you had. If you had an autologous tissue (flap) reconstruction, and there is no implant, then this wound will heal with local wound care. It will likely take weeks. If there is an implant present, I would be much more concerned as the risk of infection would be greater. If on the other hand, your surgery was a breast reduction and not a mastectomy, then you should also heal with time. Your surgeon already attempted to recluse this wound unsuccessfully so additional surgery would likely fail. It is difficult, but be patient. Once your wound heals, allow more time to soften the scar and you can consider a scar revision at a later date
