I went to see Dr. Millgrim due to extreme difficulties breathing through my nose for many years. Sleeping and working out were the most frustrating. My nose had been broken four times in the past, and I already had my septum fixed twice, in addition to going face first through a windshield, the week after my first surgery. Anyway, it took only seconds for Dr. Milgrim to look inside my nose, and I knew it was structurally damage, by the speed in which he put down the tool. Everything he said made complete sense, I just didn’t want to believe it, and I didn’t want anyone touching my nose ever again. Well, let me tell you choosing Dr. Milgrim was the absolute best decision I have ever made!!! Dr. Milgrim not only fixed all the internal structural damage from the past (which was a lot), he corrected, and perfected the outside. I now have the nose I should have always had. I look natural and symmetrical. I’m taking in more air than ever before. Also, for many years when someone wanted to take a picture I would only allow it from my right side, the better side. Now I love every side equally. The confidence I have in Dr. Milgrim is invaluable. He made me feel so comfortable and he was so informative throughout the thousands of questions I asked him. I have recommended Dr. Milgrim to a few of the girls in my office. I just can’t say enough! He is the best and anyone knows (with my history) that is a HUGE compliment. He will be the only doctor that will touch my nose for the rest of my life!!! Updated on 26 Mar 2018: I went to see Dr. Millgrim due to extreme difficulties breathing through my nose for many years. As other physicians have stated, the cost of rhinoplasty can vary depending on numerous factors, including: A. Location: Physicians located in large cities have higher rent and other expenses compared with physicians in smaller cities or rural areas. Therefore, they generally charge more for a cosmetic surgery. B. Quote: When you phone in for a quote, some practices quote only the surgeon's fee, while others quote everything: surgeon's fee, anesthesia fee, operating room fees, etc. It's wise to clarify the list of included items when you phone a plastic surgery office to ask about prices. C. Complexity: A simple rhinoplasty takes less time than a revision rhinoplasty and, therefore, may cost less. While price is definitely a consideration, the biggest issue, of course, is the quality of the result. Identifying the most skilled board certified facial plastic surgeon in your area, meeting that surgeon for a consultation, and viewing as many before/after photos as possible is the best approach. The cost of a face lift varies by multiple factors, including: 1. Your unique facial aging pattern affects the price. For example, some patients who see me in consultation at my NJ facial plastic surgery practice need a full facelift plus an eyelift and/or forehead lift, while another patient's aging pattern indicates that a mini lift (focusing primarily on the mid face, ie, cheek sagging and jowling) will meet their needs. The more work that needs to be done, the longer I'm in the OR and therefore, the higher the price to cover my time, facility time and anesthesia time. 2. The location of your procedure. Not only does this vary geographically (physicians in the Midwest pay lower rent than a NYC plastic surgeon, for example, so their prices can be lower in general because their overhead is lower), but it can vary by facility. By that I mean that in some areas a surgicenter may be lower in price than a hospital-based surgery. 3. Anesthesia. Anesthesia delivered by a board certified anesthesiologist is going to cost more than anesthesia delivered by a nurse anesthetist. The bottom line, however, is to choose a plastic surgeon who will give you the best results. A bargain isn't a bargain if you don't like the results and end up having a revision. A face lift should last 7-10 years. The difference between one practice and another is minimal when you divide that difference over 7-10 years. Rather than focusing primarily on price, focus on experience, technique (as evidenced by lots of before/after photos) and patient responses here on RealSelf and other review sites. Peepsey, I'd love to tell you that applying an electrical charge to your facial skin will give you results even remotely approach a true facelift but if it did I'd be doing it here at my NJ facial plastic surgery practice and so would every other board certified facial plastic surgeon I know. I don't offer it because (A) it hasn't been proven in studies to work and (B) I want my patients to get predictable results for the money they spend at my Teaneck plastic surgery office. I don't know how old you are, the quality of your skin or your unique facial aging pattern. However, if fillers are doing a good job in terms of lifting those areas of the face that concern you, I'm not sure what the purpose of the microcurrent facelift would be. If Botox and fillers are no longer providing the results you need, I'd recommend either laser skin rejuvenation or a chemical peel, which we "know" works, or a "true" facelift. You don't state whether you have neck aging or not, which would also be addressed in a surgical facelift. If you're committed to having this electric facelift, I'd advise not having injectables anytime within two weeks before or after electrifying yourself. I can't speak for other surgeons in terms of how they charge for revisions. In my NJ facial plastic surgery practice, patient satisfaction is THE most important thing we offer. If a patient has complied with postop instructions but has a poor result, I see him or her in consultation to uncover the problem. Without pre and postop photos and other details such as your age and skin condition, I cannot tell you whether or not the problem was created by surgical technique. However, it IS my opinion that you should have the implant removed, let the area heal and do another implant of the proper size the next time. I would not assume that "third time is a charm," however. If you cannot agree on whether or not you should pay for these revisions with your current surgeon, it may be in your best interest to have a couple consultations with other board certified facial plastic surgeons in your area to determine what may have occurred to cause this problem and what would be the best course of action to take. You are correct that a pseudo capsule will shrink, but it takes longer than 3 months, in my experience. I would not be concerned at this moment that your chin still looks like the implant is there; give it 6-12 mos before deciding whether the scar tissue is permanent.
