Thank you for your question. This is something we see frequently in our practice. Scar tissue will never be as strong as native/uncut tissue. For this reason when we perform secondary, tertiary, etc breast surgery we encourage patients to use the internal mesh, that is dissolvable over time, but initially helps to strengthen the support of the tissue to hold the implant up. Sometimes if the position of the previously lifted nipple is in the correction position but the bottom is "bottomed out" then we can do a modified lift and placement of mesh just in the inframammary fold crease to correct this issue.

The answer to this question really depends on what you're looking for in the overall appearance of your breast AFTER the surgery. If you are looking to keep it small/conservative but perky - nothing more than a moderate profile implant than more than likely you could benefit from a type of lift. However, you would also be a good candidate for the dual-plane breast augmentation using a full profile (higher volume for your breast width) implant with possible placement of internal bra to support the size while filling out your breast envelope without requiring the mastopexy (lift) scars.

When discussing the implant volume that fits you best, it is essential to base it on the geometry of your breast width. When patients come to me, I will typically measure the width of their breast in centimeters to determine the width, or diameter, of the implant that should be used to fit your body. Once we narrow that down to within 0.5-0.75 cm we look at what profile of breast the patient is interested in having to achieve a certain level of upper pole fullness. The profile choice and the diameter of the implant are what gives your surgeon and yourself a clearer picture of what volume (in ccs) will fit you best. Typically each profile up (More Cleavage/upper pole fullness) will add 45-75cc for the same diameter (width) of implant.

Thank you for this question. The amount of volume that can be places submuscular is determine by the width of your frame/muscle/breast tissue. So, being petite it may be impossible in one stage to fit that amount of volume without expanding the pocket with a smaller implant first. We hope this helps!

Thank you for this question. We get this alot in our practice. When people question volume we ask them about what they are looking for a “natural” shape? A “little more than natural” shape? Or a “wow those are fake!” Shape. This helps us determine the profile of implant you get. If you are set on a moderate profile for the more natural shape it will not make a difference other than in Width of your breast. The projection of 30cc is non-existant. In other words a moderare profile implant to go up in size projects the same. Adding “volume” is adding width. To add projection you chose a higher profile that is the same diameter that fits within YOU. That may be 60cc more but again thats only 2x full medicine cups...not alot in reality. Thank you for this question. You bring up a strong point about profile. In general a higher profile implant will be more dramatic of a change with more upper pole fullness than a moderate plus. The 50cc isn’t really such a big difference as it is basically 1.5x medicine cups of fluid. And the 0.4cm of width will not create much more side boob. Hope this helps!

Thank you for posting this question. Women with such a petite body size and breast shape may need sometimes two breast augs to stretch their skin. If your breast diameter is 11.5cm you really cant put anything wider than 10.5cm in a high profile for max volume. Most likely a high 270-295cc. Thats really stretching it. The general rule is for every 150cc you go up is 1 cup size. I’m sure someone may measure you as a c cup afterwards but maybe not all the time. Hope this helps.

Thank you for this question. I find that alot of potential patients are misguided with implants in 50cc increments. The more modern way of doing this is to use your own breast width to set the diameter of the implant and then if cleavage is your main issue talk about the PROFILE of the implant. Most of the time we ask, “Would you like to look natural, a little more than natural, or very augmented”. This sets up the profile being moderate, full, pr high profile. That will give you the VOLUME you should go with as a patient. A moderate profile (natural) for a breast width of 12.5cm may be 295-320cc. A full in the same diameter may be 345-385cc. A high profile in the same diamter may be 385-445cc. And 50cc isnt that much to discern cleavage. 50cc is 1.5x medicine cups. What you really need is profile projection to create cleavage. Thank you for your question. The tuberous breast correction really depends on the grade of tuberous breast. In your case you have a high riding Inframammary Fold and Nipple Herniatio of tissue. For this case you may not need an entire life with vertical scarring. We would typically perform a dual-plane aug with the incision lowering the fold and a periareolar reduction scar to reduce the herniated tissue and cone shape. With some radial scoring it will help round out the tissues.

Typically your surgeon should be able to tell you what size will fit within your body by measuring you breast width. There are 600 different sizes of inplants with a different size every 15cc and 5 different profiles for each. Typically if your breast measured 14cm wide you could use a 13cm wide implant and the most volume would be a high profile. This would be 500+ cc...however if, judging by your picture, you measure around 12.5-13cm breast width we could only use a 11.5-12cm diameter implant with max volume in high profile being around 440cc.
