I am 38 years old, don't smoke or tan, and generally take good care of my skin, but I wanted to get rid of some acne scars, smooth some fine lines, and firm the skin under my eyes. Don't really have crow's feet, just a little crepiness. A few of my acne scars were right in that nose-to-mouth line, so they made this line look deeper on my left side. I was considering filler injections (like Juvederm) and maybe a chemical peel. My Dr. said the Pearl Fractional would be more effective, not to mention more permanent, for the acne scars, but I didn't need it all over my face since otherwise my skin was in good condition. He also suggested the Pearl Laser rather than the medium-depth chemical peel for my under-eye and forehead wrinkles. He performs both the Pearl laser and chemical peels, and they cost the same amount, but he said the Pearl was less invasive and painful than the chemical. I am also VERY fair-skinned, which I understand can make laser procedures more effective. So this was the one time in my life that being pale worked in my favor. So he did the Pearl laser all over my face, and then the Pearl Fractional just on my cheeks. He charged me 1/2 price for the Pearl fractional since he didn't do my entire face, and discounted the price of the Pearl laser since I was having more than one procedure, which I thought was a pretty good deal. He even did the Pearl laser on my neck. I had both procedures performed with just the topical anesthetic, and had very little pain. It hurt a little during the procedures, but the pain was very brief. The worst part was later that day and night. I was told to expect some oozing from the Pearl Fractional area, but I was oozing blood from every hole they drilled in my face! Even this was NOT painful, just gross. This continued for the first 18 hours or so. My pillow was ruined, even with 2 pillow cases. I never really felt pain, my face was just warm like a sunburn. The vinegar soaks and vaseline helped. And I was a little nauseous the first night and the next day. The procedures were done on a Wednesday afternoon, my skin peeled completely on Saturday, and I returned to work, with make-up, on Monday. After the oozing stopped, it was really unbelievable how quickly I healed. My skin improved exponentially each day. It has been one week, and I look great! My cheeks are still red and my skin is drier than normal, but getting better every day. My acne scars are about 90% gone, and the skin under my eyes and the rest of my face is firmer and smooth. After spending who-knows-how-much on products that don't work, it is really satisfying to see such great results. I know this was a really long review, but I have searched extensively trying to find out what to expect with these treatments, but couldn't find anything detailed. The only cons to having these procedures I can think of are the messiness from the oozing and the vaseline, and that I was bored to death since I couldn't go out in the sun and had to stay home. Small sacrifice!
I had a terrible experience with eyelid surgery. I have large rope-like scars that hang on one eye. My lids are flat and the corners are rounded (Kenny Rogers). He has no aesthetic vision whatsoever and the community would be better served if he just stuck to taking out tonsils!
Beginning in puberty, my nose grew larger with a hump on the bridge. I had my first rhinoplasty when I was 18 years old right before graduation in 1990. I am going to have my third rhinoplasty revision surgery in October. After my first surgery, the tip began to turn to the right. I had already moved out of state and decided to live with it for the time being. In 2002, I needed sinus surgery and asked the ENT to try and fix the tip. He did. It healed fine and yes it was better/straighter. In 2009, I broke my nose in an accident. At the time, I didn't know it was broken. When it healed, it looked much the same way as before. I decided to have an upper eye lift in 2012, and consulted with a PS about a rhino revision also. I explained my history and asked for a cute, less conspicuous looking nose. In post op my dr. said there was a lot to repair from my accident and he had even used cartilage from my right ear to build the tip up as he did not have enough left from my previous surgeries. After I healed, it was straighter but had a very similar look and the tip seems to have fallen over time. I feel like if he would go smaller and scale down my nose there would be less chance of a curving in the bridge and/or the tip. I am even wondering if the asymmetry is due to my bone structure. The left side of my face is a little underdeveloped compared to the right side. You can really tell this in my jaw line, my eyebrow and my ear. Can the mid-line of my face be causing shifting and curving of my nose? Should my PS reinforce my nose with something extra? I am going to consult with him again before my surgery and ask him these questions. Is there anything else I should ask him? I have noticed in 99% of the rhinoplasty post op photos there aren't drastic changes such as in length or depth of the patients nose. Is there a reason? Am I expecting too much if i ask him to make my nose smaller? He has said by raising the tip, it will give the appearance of bring smaller. I would like the depth and length of my nose to be scaled down in addition to raising the tip. What do you think? I am also having my ears pinned this time too as my right ear sticks out from the lack of cartilage. Updated on 12 Sep 2016: I have added a picture of my nose in 8th grade before any surgeries and one after the first rhinoplasty. Updated on 15 Oct 2016: So happy I had Dr. Sanders redo my nose. I had a ton of scar tissue that was removed and he lifted the tip! I love it!
While fat can certainly be used in the nose as a filler, in your case, it would depend on the amount of bowing present and whether there is a structural problem or not. If there is a structural problem then cartilage would be a better answer. Keep in mind that fat is not usually a one time procedure. It usually needs to be repeated a few times for the permanent end result
not only can it be sewn together it should be repaired ASAP. the healing process can cause a significant deformity of the nostril if the cartilage is not repaired. "notching" is common with an injury like this but can be avoided.
a tricophytic lift is what you are looking for. the result should give a nice lift to brow and an effective lowering of the hairline by 1-2 cm...cost is a bit more in my office because of the much more tedious closure needed over the usual endoscopic brow lift. plan on using bangs for a couple months while the scars mature.
your smile appears to be affected for sure. this is likely due to the "bunny" and lip lines. the bunny lines are improved by injecting the nasalis muscles. sometimes the muscle near the nasalis can be injected (levator of the lip) this along with too much in the lip itself can give an immobile lip...this is a 3-4 month problem. there is no way to make it go away. secondly the chin is an area i inject all the time but again precision is key...if you are off line slightly it will weaken a depressor of the corner of the mouth. bottom line is this injector is missing the mark on dose or area or both.
This would depend on the donor area which we cannot really see from the pictures but likely is good. You should be doing all of the "medical" therapies as well 1)minoxidil twice daily 2) propecia 3) laser 4) supplements. Lastly, I would consider PRP injections for someone your age and hair loss pattern
