Yes, a CT scan would show the bone structure. Also, 4.1 refers to the size of the implant and is a very common size.

The new DDS will have to know some information about the implant. As long as it is healthy and doing okay they will be able to complete the implant.

It is possible that you are hitting the tooth too much and causing slight trauma to the tooth. I suggest going to see your dentist to have him or her check on your bite.
