I recommend you see a gynecologist who performs labiaplasty and who is familiar with fistulas for an exam and evaluation.  These are likely two separate problems, but a thorough and proper exam will help show the diagnosis.  It may be suture or scars or other processes.  There are ways to diagnose fistulas based on what the exam findings are and your symptoms.  A thorough gynecologist will be able to diagnose the problem and recommend the proper treatment.
